【摘要】：背景:富血小板血漿因含有大量血小板源性生長(zhǎng)因子對(duì)膝骨性關(guān)節(jié)炎有較好的療效,但目前對(duì)關(guān)節(jié)面軟骨的修復(fù)效果尚有爭(zhēng)議。目的:觀察富血小板血漿對(duì)兔膝骨關(guān)節(jié)炎模型的關(guān)節(jié)軟骨修復(fù)作用。方法:采用Hulth法建立右膝骨關(guān)節(jié)炎兔模型,分別于右膝關(guān)節(jié)腔灌注富血小板血漿和玻璃酸鈉,并設(shè)置模型組和假手術(shù)組作對(duì)比。造模后8周,測(cè)定各組兔關(guān)節(jié)滑液中一氧化氮水平,觀察關(guān)節(jié)面形態(tài)及軟骨修復(fù)情況。結(jié)果與結(jié)論:(1)關(guān)節(jié)面軟骨形態(tài)比較:富血小板血漿治療組關(guān)節(jié)面損傷修復(fù)情況明顯好于其他3組;(2)富血小板血漿治療組軟骨組織Mankin’s評(píng)分和關(guān)節(jié)液中一氧化氮水平均低于模型組和玻璃酸鈉治療組(P0.05),高于假手術(shù)組(P0.05);(3)結(jié)果說(shuō)明:膝關(guān)節(jié)腔灌注富血小板血漿對(duì)兔膝骨關(guān)節(jié)炎模型的關(guān)節(jié)軟骨修復(fù)效果較好,效果優(yōu)于玻璃酸鈉灌注。
[Abstract]:Background: platelet-rich plasma containing a large number of platelet-derived growth factors has a good effect on knee osteoarthritis, but the repair effect of articular cartilage is still controversial. Objective: to observe the effect of platelet-rich plasma on articular cartilage repair in rabbit knee osteoarthritis model. Methods: the rabbit model of right knee osteoarthritis was established by Hulth method. Platelet-rich plasma and sodium hyaluronate were perfused into the right knee joint cavity respectively. After 8 weeks, the levels of nitric oxide in synovial fluid of rabbits were measured, and the morphology of articular surface and cartilage repair were observed. Results and conclusion: (1) the morphology of articular surface cartilage: the repair of articular surface injury in the platelet-rich plasma treatment group was better than that in the other three groups, (2) the Mankinsscale of cartilage tissue and the nitric oxide in articular fluid in the platelet-rich plasma treatment group were significantly better than those in the other three groups. The levels were lower than those in the model group and sodium hyaluronate treatment group (P0.05), and higher than those in the sham-operated group (P0.05); (3). The results showed that platelet-rich plasma perfusion into the knee joint cavity had a better effect on the repair of articular cartilage in rabbit knee osteoarthritis model. The effect was better than sodium hyaluronate infusion.
