【摘要】：[目的]探討雙側接骨板治療復雜肱骨髁間骨折的臨床效果。[方法]納入67例復雜肱骨髁間骨折患者,隨機分為兩組,接骨板組32例,克氏針組35例,分別實施雙側接骨板修復治療與克氏針內固定治療。術后隨訪12個月,觀察肘關節(jié)功能恢復情況、肘關節(jié)活動度變化以及不良反應和并發(fā)癥發(fā)生情況。[結果]接骨板組的手術時間顯著長于克氏針組,但術后骨折愈合時間顯著短于克氏針組,P0.05。術后隨訪12個月,接骨板組的肘關節(jié)功能優(yōu)良率為90.63%,顯著高于克氏針組的77.14%,P0.05。術后1、6、12個月對兩組肘關節(jié)活動度進行評估,術后不同時間,兩組的肘關節(jié)活動度均顯著高于本組術前,P0.05;且術后不同時間組間比較,接骨板組活動度均顯著大于克氏針組,P0.05。兩組的術后不良反應發(fā)生率,克氏針組顯著高于接骨板組,P0.05。[結論]較克氏針內固定治療,雙側接骨板修復治療復雜肱骨髁間骨折可以獲得更好的臨床修復效果,促進患者肘關節(jié)功能和活動度的提高,并減少不良反應的出現。
[Abstract]:[objective] to investigate the clinical effect of bilateral plate in the treatment of complex humeral intercondylar fracture. [methods] Sixty-seven patients with complex humeral intercondylar fracture were randomly divided into two groups: the plate group (n = 32) and Kirschner needle group (n = 35). After 12 months of follow-up, the recovery of elbow function, the changes of elbow motion, adverse reactions and complications were observed. [results] the operative time in the plate group was significantly longer than that in the Kirschner needle group, but the fracture healing time was significantly shorter in the Yu Ke needle group (P 0.05). After 12 months follow-up, the excellent and good rate of elbow joint function in the plate group was 90.63, which was significantly higher than that in the Kirschner needle group (77.14 and P 0.05). The elbow motion of the two groups was evaluated at 6 and 12 months postoperatively. At different time after operation, the motion of elbow joint in the two groups was significantly higher than that in the preoperative group (P 0.05), and that in the plate group was significantly higher than that in the Kirschner needle group (P 0.05). The incidence of postoperative adverse reactions in the two groups was significantly higher in the Kirschner needle group than in the plate group (P 0.05). [conclusion] compared with Kirschner wire internal fixation, bilateral plate repair for complex humeral intercondylar fracture can obtain better clinical repair effect, promote the function and mobility of elbow joint, and reduce the occurrence of adverse reactions.
