微輻射技術(shù)在導(dǎo)航引導(dǎo)經(jīng)皮椎體成形術(shù)中的應(yīng)用

【摘要】:目的探討微輻射技術(shù)在導(dǎo)航引導(dǎo)經(jīng)皮椎體成形術(shù)(PVP)中減少輻射的效果。方法通過3項(xiàng)手術(shù)器械(體表椎體定位器、經(jīng)皮導(dǎo)航動態(tài)參考架、導(dǎo)航專用穿刺針)的研制和2項(xiàng)引進(jìn)技術(shù)(雙鉛板和骨水泥液壓輸送)的應(yīng)用,與現(xiàn)有導(dǎo)航引導(dǎo)下PVP進(jìn)行對比。因單椎體骨折行PVP患者80例,隨機(jī)分為改良組和對照組各40例,改良組使用通過3項(xiàng)手術(shù)器械研制和2項(xiàng)引進(jìn)技術(shù)的應(yīng)用得以改良的微輻射技術(shù),對照組使用目前國內(nèi)醫(yī)院常用的導(dǎo)航引導(dǎo)下PVP,術(shù)后隨訪12個(gè)月,觀察分析手術(shù)前后VAS評分、平均手術(shù)時(shí)間和術(shù)者暴露劑量。結(jié)果 80例椎體成功注射骨水泥,無嚴(yán)重并發(fā)癥,術(shù)后隨訪時(shí)間8~24個(gè)月,平均16個(gè)月,所有患者手術(shù)前后VAS評分均有顯著差異,兩組間差異不明顯。改良組平均手術(shù)時(shí)間少于對照組,術(shù)者暴露劑量明顯低于對照組(P0.01)。結(jié)論采用微輻射技術(shù)的導(dǎo)航引導(dǎo)下PVP,能有效縮短手術(shù)時(shí)間,特別是能明顯減少術(shù)者暴露劑量,大大提高了需反復(fù)承受醫(yī)療輻射術(shù)者的人身安全。
[Abstract]:Objective to evaluate the effect of microradiation in navigation guided percutaneous vertebroplasty (PVP). Methods through the development of three kinds of surgical instruments (body surface vertebrae positioner, percutaneous navigation dynamic reference frame, navigation special puncture needle) and the application of two imported techniques (double lead plate and bone cement hydraulic transportation). Compared with PVP under existing navigation guidance. Eighty patients with PVP due to single vertebral body fracture were randomly divided into two groups: the modified group (n = 40) and the control group (n = 40). Patients in control group were followed up for 12 months under the guidance of navigation, and VAS scores, mean operating time and exposure dose were observed and analyzed before and after operation. Results 80 cases of vertebral body were injected with bone cement successfully without serious complications. The follow-up time was 8 ~ 24 months (mean 16 months). There was significant difference in VAS score between the two groups before and after operation, and there was no significant difference between the two groups. The average operation time of the modified group was less than that of the control group, and the exposure dose of the operation group was significantly lower than that of the control group (P0.01). Conclusion under the guidance of microradiation, PVP can effectively shorten the operation time, especially reduce the exposure dose of the operator, and greatly improve the safety of the patients who need repeated medical radiation.
