骨碎補(bǔ)總黃酮聯(lián)合全身振動(dòng)對絕經(jīng)后骨質(zhì)疏松性椎體壓縮性骨折術(shù)后的影響

【摘要】:目的評價(jià)骨碎補(bǔ)總黃酮聯(lián)合全身振動(dòng)(whole-body vibration,WBV)對絕經(jīng)后骨質(zhì)疏松性椎體壓縮性骨折PVP術(shù)后的影響。方法 129例的絕經(jīng)后骨質(zhì)疏松性椎體壓縮性骨折納入本研究,隨機(jī)分為骨碎補(bǔ)總黃酮組、全身振動(dòng)組及聯(lián)合治療組,所有患者均接受經(jīng)皮椎體成形術(shù)(PVP)治療;骨碎補(bǔ)總黃酮組術(shù)后給予骨碎補(bǔ)總黃酮治療,全身振動(dòng)組術(shù)后給予全身振動(dòng)治療;聯(lián)合治療組術(shù)后給予骨碎補(bǔ)總黃酮聯(lián)合全身振動(dòng)治療,治療為期6個(gè)月。對3組患者治療前后的疼痛VAS評分、腰椎及髖部骨密度、血清總堿性磷酸酶(alkaline phosphatase,ALP)、血清Ⅰ型膠原羧基端肽交聯(lián)(carboxy-telopeptide of type I collagen,β-CTX)及血鈣(Ca)、磷(P)變化情況進(jìn)行對比,同時(shí)對患者服藥后的不良反應(yīng)發(fā)生率進(jìn)行比較。結(jié)果治療6個(gè)月后3組患者VAS評分均較治療前有明顯改善(P0.05),且聯(lián)合治療組患者評分明顯低于骨碎補(bǔ)總黃酮組及全身振動(dòng)組(P0.05)。治療6個(gè)月后,3組患者骨密度較治療前顯著改善(P0.05),血清Ca、P、ALP和β-CTX較治療前有顯著改變(P0.05),且聯(lián)合治療組均明顯優(yōu)于骨碎補(bǔ)總黃酮組及全身振動(dòng)組(P0.05)。3組藥品不良反應(yīng)發(fā)生率比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論骨碎補(bǔ)總黃酮聯(lián)合全身振動(dòng)降低絕經(jīng)后骨質(zhì)疏松椎體壓縮性骨折PVP術(shù)后VAS評分,且能改善髖部腰椎骨密度,安全性較高,值得在臨床中推廣應(yīng)用。
[Abstract]:Objective to evaluate the effect of whole-body VibrationWBV on postmenopausal osteoporotic vertebral compression fracture. Methods 129 cases of postmenopausal osteoporotic vertebral compression fracture were randomly divided into three groups: total flavonoids group, whole body vibration group and combined treatment group. All the patients were treated with percutaneous vertebroplasty (PVP). The total flavone group was treated with total flavone after operation, the whole body vibration group was treated with whole body vibration after operation, and the combined treatment group was treated with total flavone and whole body vibration for 6 months. VAS scores, bone mineral density of lumbar spine and hip, serum alkaline phosphatase, carboxy-telopeptide of type I collagenase (尾 -CTX), serum calcium (Ca) and phosphorus (P) were compared before and after treatment. At the same time, the incidence of adverse reactions after taking medicine was compared. Results after 6 months of treatment, the VAS scores in the three groups were significantly improved compared with those before treatment (P0.05), and the scores in the combined treatment group were significantly lower than those in the total flavonoids group and the whole body vibration group (P0.05). After 6 months of treatment, the bone mineral density (BMD) of the three groups was significantly improved than that of the control group (P0.05), and the serum levels of Catropicin ALP and 尾 -CTX were significantly changed (P0.05), and the incidence of adverse drug reactions in the combined treatment group was significantly better than that in the total flavonoid group and the whole body vibration group (P0.05), and the incidence of adverse drug reactions in the combined treatment group was significantly higher than that in the whole body vibration group (P0.05). The difference was not statistically significant (P0.05). Conclusion Total flavonoids combined with whole body vibration can reduce the VAS score of postmenopausal osteoporotic vertebral compression fracture after PVP and can improve the bone mineral density of hip lumbar vertebrae. It is safe and worthy to be popularized in clinic.
