通氣側(cè)肺吸入伊洛前列素對肺動脈壓和缺氧性肺血管收縮的影響

【摘要】:目的 探討霧化吸入伊洛前列素對大鼠單肺通氣(one-lung ventilation,OLV)期間肺動脈壓和缺氧性肺血管收縮(hypoxic pulmonary vasoconstriction,HPV)的影響。方法 雄性SD大鼠30只,隨機分為A、B、C三組,每組10只。原位離體肺灌注模型建立后,調(diào)整氣管導(dǎo)管深度致左側(cè)肺OLV,FiO_2為100%,打開連接于呼吸回路的霧化器,A組吸入生理鹽水,B組吸入伊洛前列素0.05μg·kg-1·min-1,C組吸入伊洛前列素0.1μg·kg~(-1)·min~(-1),記錄灌注10min(T_1)、霧化吸入10min(T_2)和OLV 1h(T_3)的平均肺動脈壓(MPAP)及左心房引流液氧分壓(PaO_2)。通過測量T_1、T_2和T_3時的左心房引流液PaO_2計算氧合指數(shù)(oxygenation index,OI)。實驗結(jié)束分別取雙側(cè)肺組織進行電鏡檢查。結(jié)果 與T_1時比較,T_2、T_3時三組MPAP明顯升高(P0.05);T_2、T_3時B、C組MPAP明顯低于A組(P0.05),且C組MPAP明顯低于B組(P0.05)。與T_1時比較,T_2、T_3時三組OI明顯降低(P0.05);T_2、T_3時C組OI明顯高于B組(P0.05)。相對于B、C組通氣側(cè),A組通氣側(cè)與A、B、C組非通氣側(cè)Ⅱ型肺泡上皮細(xì)胞核膜外突內(nèi)陷,部分板層小體排空。結(jié)論 在大鼠原位肺灌注模型中,單肺通氣期間霧化吸入伊洛前列素能明顯降低平均肺動脈壓,減少肺內(nèi)分流并增加氧合。
[Abstract]:Objective to investigate the effects of aerosol inhalation of iloprost on pulmonary artery pressure and hypoxic pulmonary vasoconstriction during one-lung ventilation in rats. Methods Thirty male Sprague-Dawley rats were randomly divided into three groups, 10 rats in each group. After the establishment of in situ isolated lung perfusion model, Adjusting the depth of trachea catheter to induce left lung OLVFiO2 is 100g.
