利用Myrian系統(tǒng)對胰腺周圍主要血管進行3D重建

【摘要】:目的探討利用Myrian系統(tǒng)構(gòu)建胰腺周圍主要血管的3D結(jié)構(gòu)的方法及其在臨床應用中的意義。方法通過GE64層CT采集60例胰腺部位掃描數(shù)據(jù),利用Myrian軟件系統(tǒng)對胰腺周圍動脈血管群、靜脈血管群的分布情況進行3D重建,并測量血管長度、夾角、匯入情況等數(shù)據(jù)。結(jié)果腹腔的主要動脈,如腹主動脈、脾動脈、肝總動脈,以及腹腔的部分靜脈,如門靜脈、脾靜脈、腸系膜上靜脈的重建率可達100%;門靜脈干長度平均(46.37±11.48)mm,其中大于45mm的優(yōu)勢長度占45.54%;胰后干長度為(34.88±9.13)mm,其中大于35mm的優(yōu)勢長度占57.21%。門靜脈與脾靜脈的夾角為銳角的有17例(占28.33%),為直角的有11例(占18.33%),為鈍角的有32例(占53.33%)。腸系膜下靜脈的匯入方式為脾靜脈的有19例(占31.67%),為腸系膜上靜脈的有34例(占56.67%),為脾靜脈和腸系膜上靜脈交匯點的有7例(占11.67%)。結(jié)論 Myrian系統(tǒng)可用于對胰腺周圍血管的3D結(jié)構(gòu)重建,而且重建率較高,可用于指導臨床手術(shù)實施的可行性分析,減少手術(shù)過程中因不清楚血管分布情況而導致的出血風險。
[Abstract]:Objective to explore the method of constructing 3D structure of major peripancreatic vessels using Myrian system and its clinical significance. Methods the scanning data of 60 cases of pancreas were collected by GE 64 slice CT. The distribution of peripancreatic artery group and venous vessel group were reconstructed by Myrian software system, and the data of vessel length, angle and confluence were measured. Results the main arteries of abdominal cavity, such as abdominal aorta, splenic artery, common hepatic artery, and partial vein of abdominal cavity, such as portal vein and splenic vein, The average length of portal vein was (46.37 鹵11.48) mm, in which the dominant length was greater than 45mm (45.54), and the length of posterior pancreatic trunk was (34.88 鹵9.13) mm, in which the superior length was larger than 35mm (57.21) mm. There were 17 cases (28.33%) with acute angle between portal vein and splenic vein, 11 cases (18.33%) with right angle and 32 cases (53.33%) with obtuse angle. There were 19 cases (31.67%) of inferior mesenteric vein, 34 cases (56.67%) of superior mesenteric vein and 7 cases (11.67%) of splenic vein and superior mesenteric vein. Conclusion Myrian system can be used to reconstruct 3D structure of peripancreatic vessels, and the reconstruction rate is high. Myrian system can be used to guide the feasibility analysis of clinical operation and reduce the risk of hemorrhage caused by unclear vascular distribution during operation.
