右美托咪定對肺癌術(shù)后患者微循環(huán)的影響

【摘要】:目的觀察2種劑量右美托咪定(Dex)對肺癌根治術(shù)后機械通氣患者微循環(huán)的影響。方法選取肺癌術(shù)后入重癥監(jiān)護室行機械通氣的患者60例,按隨機數(shù)字表法分為A、B兩組,每組30例,分別靜脈微泵0.6和0.8μg/(kg·h)劑量的Dex。觀察并記錄給藥前(T_0)和給藥后15 min(T_1)、30 min(T_2)、1 h(T_3)、2 h(T_4)的心率(HR)、收縮壓(SBP)、舒張壓(DBP)、心輸出量(CO)、經(jīng)皮氧分壓(Tc PO_2)、經(jīng)皮二氧化碳分壓(Tc PCO_2)、氧分壓(Pa O_2)、二氧化碳分壓(Pa CO_2)及血乳酸(Lac)值。記錄心血管不良事件的發(fā)生情況。結(jié)果兩組不同時間血流動力學(xué)指標(biāo)有差異,均呈下降趨勢,以B組更明顯,兩組組間各指標(biāo)比較,差異無統(tǒng)計學(xué)意義(P0.05);兩組不同時間Pa O_2、Pa CO_2及Tc PCO_2比較,差異無統(tǒng)計學(xué)意義(P0.05);兩組不同時間Lac比較,差異有統(tǒng)計學(xué)意義(P0.05),呈上升趨勢。兩組不同時間Tc PO_2比較,差異有統(tǒng)計學(xué)意義(P0.05),兩組給藥后變化趨勢比較,差異有統(tǒng)計學(xué)意義(P0.05),兩組Tc PO_2均較用藥前升高。結(jié)論 0.6μg/(kg·h)Dex泵注肺癌術(shù)后機械通氣患者,血流動力學(xué)更穩(wěn)定,微循環(huán)改善更顯著。
[Abstract]:Objective to observe the effect of two doses of dexmetomidine (Dex) on microcirculation in patients with mechanical ventilation after radical resection of lung cancer. Methods Sixty patients with lung cancer received mechanical ventilation in intensive care unit after operation were randomly divided into two groups: group A (n = 30) and group A (n = 30) with intravenous micropump of 0.6 渭 g / (kg h) and 0.8 渭 g / (kg h) respectively. The heart rate (HR), systolic blood pressure (SBP), diastolic pressure (DBP), cardiac output (CO), percutaneous partial oxygen pressure (Tc PCO2), partial pressure of oxygen (O 2), partial pressure of carbon dioxide (CO 2), partial pressure of CO 2 (CO 2), partial pressure of oxygen (CO 2), partial pressure of CO 2 (CO 2) and values of blood lactate (Lac) were observed and recorded before and 15 min after administration. Results the hemodynamic indexes of the two groups were different at different time and showed a downward trend, especially in group B, and there was no significant difference between the two groups (P 0.05), and there was no significant difference between the two groups (P0.05), and there was no significant difference between the two groups at different time points (P < 0.05). Conclusion 0.6 渭 g / (kg h) Dex pump has more stable hemodynamics and improved microcirculation in patients with lung cancer after mechanical ventilation.
