【摘要】：目的探討硬膜外和全身麻醉兩種方式對(duì)老年高血壓病患者術(shù)后麻醉恢復(fù)期的影響效果。方法行手術(shù)治療的178例患者,根據(jù)麻醉方式不同分為全麻組(89例)和硬膜外組(89例),全麻組患者予以全身麻醉,硬膜外組予以全身麻醉復(fù)合硬膜外麻醉,比較兩組患者麻醉效果、心率、血壓異常情況、術(shù)后拔管時(shí)間、蘇醒時(shí)間及術(shù)后對(duì)患者認(rèn)知功能的影響。結(jié)果硬膜外組患者麻醉優(yōu)良率明顯高于全麻組(P0.05);麻醉前,全麻組和硬膜外組患者血壓和心率異常率比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),麻醉后全麻組血壓、心率異常率顯著高于硬膜組(P0.05);硬膜外組患者術(shù)后認(rèn)知功能障礙發(fā)生率明顯低于全麻組(P0.05),術(shù)后拔管時(shí)間和蘇醒時(shí)間顯著短于全麻組(P0.05)。結(jié)論硬膜外麻醉對(duì)老年高血壓手術(shù)治療患者具有較好的麻醉效果,不僅能有效維持血壓和心率正常,還能縮短術(shù)后蘇醒時(shí)間,減少對(duì)認(rèn)知功能障礙的影響。
[Abstract]:Objective to investigate the effect of epidural and general anesthesia on the recovery of postoperative anesthesia in elderly patients with hypertension. Methods 178 patients were divided into general anesthesia group (89 cases) and epidural group (89 cases) according to different anesthetic methods. The patients in general anesthesia group were given general anesthesia, and general anesthesia combined with epidural anesthesia was given in epidural group. The effects of anesthesia, abnormal heart rate, blood pressure, extubation time, recovery time and postoperative cognitive function were compared between the two groups. Results the excellent and good rate of anesthesia in epidural group was significantly higher than that in general anesthesia group (P0.05), there was no significant difference in abnormal blood pressure and heart rate between general anesthesia group and epidural group before anesthesia (P0.05). The abnormal rate of heart rate was significantly higher than that of the epidural group (P0.05), the incidence of postoperative cognitive dysfunction in the epidural group was significantly lower than that in the general anesthesia group (P0.05), and the time of extubation and recovery after operation was significantly shorter than that in the general anesthesia group (P0.05). Conclusion epidural anesthesia can not only maintain normal blood pressure and heart rate, but also shorten the recovery time and reduce the influence on cognitive dysfunction in elderly patients with hypertension.
