【摘要】：目的構(gòu)建術(shù)中甲狀旁腺評(píng)分系統(tǒng),初步驗(yàn)證該評(píng)分系統(tǒng)與術(shù)后甲狀旁腺功能減退的關(guān)系,并通過(guò)該評(píng)分系統(tǒng)預(yù)測(cè)術(shù)后甲狀旁腺的功能。方法建立術(shù)中原位甲狀旁腺評(píng)分系統(tǒng)及甲狀旁腺移植評(píng)分系統(tǒng),回顧性分析2014年07月至2014年12月在我院甲狀腺外科同一治療組接受手術(shù)治療并經(jīng)病理證實(shí)的甲狀腺腫瘤患者137例。男性患者22例(16.06%).女性患者115例(83.94%)。患者年齡在18~73歲之間,平均(44.67士12.42)歲。監(jiān)測(cè)術(shù)前甲狀旁腺激素(parathyroid hormone PTH)、血鈣值及術(shù)后12-24小時(shí)、術(shù)后2周的血PTH、血鈣值,根據(jù)術(shù)后2周的血清甲狀旁腺激素(PTH2)于術(shù)前血清甲狀旁腺激素(PTH0)的比值,來(lái)評(píng)估甲狀旁腺的功能受損情況。根據(jù)上述的甲狀旁腺評(píng)分表,最后可以得出經(jīng)過(guò)手術(shù)治療后的甲狀旁腺評(píng)分差值。采用統(tǒng)計(jì)學(xué)軟件IBM SPSS 19.0對(duì)數(shù)據(jù)進(jìn)行分析,計(jì)數(shù)資料采用χ2檢驗(yàn)比較分析,計(jì)量資料采用t檢驗(yàn),規(guī)定P0.05時(shí)具有統(tǒng)計(jì)學(xué)差異。將術(shù)后兩周的血清甲狀旁腺激素與術(shù)前的血清甲狀旁腺激素的比值(PTH2/PTH0)定為Y軸,把甲狀旁腺評(píng)分的差值定位X軸,描繪二者的散點(diǎn)圖,利用IBM SPSS19.0統(tǒng)計(jì)軟件對(duì)兩者進(jìn)行線(xiàn)性回歸分析,規(guī)定P0.05時(shí)兩者線(xiàn)性相關(guān)。使用IBM SPSS19.0統(tǒng)計(jì)軟件的受試者工作特征曲線(xiàn)(receiver operator characteristic curve,ROC曲線(xiàn))過(guò)程。以甲狀旁腺評(píng)分差值為檢驗(yàn)變量(Test Variable),術(shù)后2周血清甲狀旁腺激素定義甲狀旁腺功能減退的結(jié)果為狀態(tài)變量(State Variable),作ROC曲線(xiàn)分析,規(guī)定P0.05時(shí)具有統(tǒng)計(jì)學(xué)差異。結(jié)果1.甲狀腺術(shù)后2周的血PTH、血鈣與術(shù)前的血PTH、血鈣進(jìn)行t檢驗(yàn),有統(tǒng)計(jì)學(xué)意義(P0.05)。2.術(shù)后兩周的血清甲狀旁腺激素與術(shù)前的血清甲狀旁腺激素的比值(PTH2/PTH0)與甲狀旁腺評(píng)分的差值進(jìn)行線(xiàn)性回歸分析,結(jié)果提示具有相關(guān)性,其中R=0.29,P=0.001。3.ROC曲線(xiàn)的曲線(xiàn)下面積為0.67,當(dāng)評(píng)分差值為8.75分時(shí),敏感度為0.81,特異度為0.57。結(jié)論1.甲狀腺手術(shù)勢(shì)必會(huì)對(duì)甲狀旁腺的功能產(chǎn)生影響。2.術(shù)中甲狀旁腺評(píng)分系統(tǒng)經(jīng)過(guò)統(tǒng)計(jì)學(xué)檢驗(yàn),具有統(tǒng)計(jì)學(xué)意義,對(duì)患者術(shù)后的甲狀旁腺功能狀態(tài)的預(yù)測(cè)具有診斷價(jià)值。
[Abstract]:Objective to establish an intraoperative parathyroid score system, and to preliminarily verify the relationship between the scoring system and postoperative parathyroid dysfunction, and to predict the function of parathyroid glands after the operation. Methods the orthotopic parathyroid score system and parathyroid transplantation score system were established. A retrospective analysis of the parathyroid parathyroid score system from 07 months to December 2014 2014 was conducted. In the same treatment group, 137 cases of thyroid tumor confirmed by operation and pathology were treated with the same treatment group. 22 cases (16.06%) were male patients and 115 cases (83.94%) of female patients. The patients were aged between 18~73 years, average (44.67, 12.42) years old. Monitoring pre operation parathyroid hormone (parathyroid hormone PTH), blood calcium value and 12-24 hours after operation, Blood PTH, blood calcium, and the ratio of parathyroid hormone (PTH2) to parathyroid parathyroid hormone (PTH0) at 2 weeks after operation to evaluate the function damage of parathyroid glands. According to the above parathyroid score table, the difference of parathyroid parathyroid score after surgical treatment was obtained. Statistical software IBM was used. The data were analyzed by SPSS 19, the count data were compared with the x 2 test, and the measurement data were examined by T, and the statistical difference was stipulated in P0.05. The ratio of serum parathyroid hormone to the preoperative serum parathyroid hormone (PTH2/PTH0) was defined as the Y axis, and the difference between the parathyroid score was defined as the X axis, and the two were depicted by the difference value of the parathyroid parathyroid hormone after the two weeks of operation. Scatter plot, using IBM SPSS19.0 statistical software to analyze both linear regression, and the linear correlation between P0.05 and IBM SPSS19.0 statistics software (receiver operator characteristic curve, ROC curve) process. The parathyroid score difference is the test variable (Test Variable), 2 weeks after the operation The result of parathyroid hormone definition of parathyroid hypogonadism was the state variable (State Variable), the ROC curve was analyzed, and the statistical difference was stipulated in the P0.05. Results 1. the blood PTH, the blood calcium and the preoperative PTH, the blood calcium were tested by T, and the serum parathyroid hormone and the operation two weeks after the operation of P0.05.2. after the operation of the thyroid gland were statistically significant. The difference between the ratio of parathyroid hormone (PTH2/PTH0) and parathyroid score was linear regression analysis. The results suggested that there was a correlation. The area under the curve of R=0.29, P=0.001.3.ROC curve was 0.67, when the score difference was 8.75, the sensitivity was 0.81, and the specificity was 0.57. conclusion 1. thyroid surgery was bound to be side to the side of the thyroid gland. The function of the glands affects the parathyroid scoring system in.2., which is statistically significant, and has a diagnostic value for the prediction of parathyroid function after operation.
