【摘要】：目的:探討股骨髁部骨折LISS微創(chuàng)系統(tǒng)(limited invasive Stabilization system,LISS)內(nèi)固定后的康復(fù)功能鍛煉方式。方法:自2012年9月至2014年12月,我院應(yīng)用LISS鋼板治療股骨髁部骨折30例,男17例,女13例;平均年齡54歲(18-82歲)。其中包含車禍傷20例、高處墜落傷6例,重物砸傷4例;開放性損傷3例,閉合性損傷27例。手術(shù)采用微創(chuàng)小切口復(fù)位,經(jīng)骨膜外與肌肉間插入鋼板,經(jīng)皮小切口將帶鎖定頭的螺釘與鋼板的螺孔單皮內(nèi)固定,術(shù)后14天之內(nèi)行膝關(guān)節(jié)CPM機(jī)康復(fù)功能鍛煉。結(jié)果:患者術(shù)后1年內(nèi)定期隨訪,骨折全部臨床愈合。膝關(guān)節(jié)功能按患者主觀感受及客觀檢查結(jié)果,以Kolment評(píng)分法進(jìn)行評(píng)估。結(jié)果顯示優(yōu)13例,良6例,可1例,差l例。優(yōu)良率90.5%。所有病例切口愈合良好,內(nèi)固定無(wú)斷裂,無(wú)感染等情況發(fā)生。結(jié)論:LISS內(nèi)固定系統(tǒng)設(shè)計(jì)優(yōu)良獨(dú)特,可保護(hù)骨膜及骨折部位的血運(yùn),CPM機(jī)可以防止關(guān)節(jié)僵硬、促進(jìn)關(guān)節(jié)軟骨的修復(fù)、提高關(guān)節(jié)活動(dòng)度,二者結(jié)合使用可以促進(jìn)骨折愈合和膝關(guān)節(jié)功能的恢復(fù),值得臨床推廣。
[Abstract]:Objective: to investigate the rehabilitation functional exercise of femoral condylar fracture after (limited invasive stabilization system (Liss) internal fixation. Methods: from September 2012 to December 2014, 30 patients (17 males and 13 females) with femoral condylar fractures were treated with Liss plate, with an average age of 54 years (18-82 years). There were 20 cases of traffic accident injury, 6 cases of falling injury, 4 cases of heavy object injury, 3 cases of open injury and 27 cases of closed injury. Minimally invasive small incision was used for reduction, plate was inserted between the periosteum and muscle, and the screw with locking head and the screw hole of the plate were fixed with single skin through the percutaneous small incision. The rehabilitation function of knee joint CPM was performed within 14 days after operation. Results: the patients were followed up regularly within 1 year after operation, all the fractures healed clinically. Knee function was evaluated by Kolment score according to subjective feeling and objective examination results. The results showed excellent in 13 cases, good in 6 cases, fair in 1 case and poor in 1 case. The excellent and good rate is 90.5%. All cases had good wound healing, no rupture of internal fixation and no infection. ConclusionThe design of the WLISS internal fixation system is excellent and unique. CPM can protect the periosteum and fracture site with CPM, which can prevent joint stiffness, promote the repair of articular cartilage, and improve the range of joint motion. Combined use of the two can promote fracture healing and the recovery of knee joint function, which is worthy of clinical promotion.
