【摘要】：目的:探討關(guān)節(jié)鏡下應(yīng)用outside-in技術(shù)修復(fù)半月板前角損傷的縫合方法及其療效。材料與方法:對(duì)沈陽(yáng)市骨科醫(yī)院2014年9月至2016年5月間的36例半月板前角損傷的患者,行關(guān)節(jié)鏡下outside-in技術(shù)修復(fù)半月板前角撕裂。對(duì)于合并前交叉韌帶斷裂的患者取自體乆繩肌腱重建前交叉韌帶。術(shù)前及術(shù)后應(yīng)用Lysholm評(píng)分、國(guó)際膝關(guān)節(jié)文獻(xiàn)委員會(huì)膝關(guān)節(jié)評(píng)估表(IKDC)以及疼痛視覺(jué)模擬評(píng)分(VAS)對(duì)手術(shù)療效進(jìn)行評(píng)價(jià)。通過(guò)Barrett標(biāo)準(zhǔn)、單腿跳遠(yuǎn)測(cè)試進(jìn)行臨床客觀(guān)評(píng)定。結(jié)果:1.所有患者隨訪(fǎng)9~29個(gè)月,平均19.1個(gè)月;2.根據(jù)Barrett標(biāo)準(zhǔn)評(píng)價(jià)半月板臨床愈合情況,33例臨床愈合,臨床愈合率為91.67%(33/36);3.術(shù)后末次隨訪(fǎng)患肢單腿跳遠(yuǎn)測(cè)試距離為(136.67±17.45)cm,33例患者達(dá)到正常肢體的對(duì)稱(chēng)性;4.術(shù)后Lysholm評(píng)分、IKDC評(píng)分及VAS評(píng)分均明顯高于術(shù)前,Lysholm評(píng)分:術(shù)前(63.72±9.81)分,術(shù)后(86.36±7.91)分;IKDC評(píng)分:術(shù)前(45.08±6.78)分,術(shù)后(89.61±4.83)分;VAS評(píng)分:術(shù)前(5.19±0.98)分,術(shù)后(0.75±0.87)分,差異均有統(tǒng)計(jì)學(xué)意義(P0.05);5.術(shù)后Lysholm評(píng)分總優(yōu)良率為91.7%。結(jié)論:關(guān)節(jié)鏡下outside-in技術(shù)修復(fù)半月板前角損傷,方法簡(jiǎn)單,不需要特殊器械,并發(fā)癥少,安全可靠,臨床愈合率高,術(shù)后患者滿(mǎn)意率高,療效顯著。
[Abstract]:Objective: to study the suture method and curative effect of arthroscopic outside-in technique for meniscus anterior horn injury. Materials and methods: from September 2014 to May 2016, 36 patients with meniscus anterior angle injury were treated with arthroscopic outside-in technique to repair anterior meniscus tear. Anterior cruciate ligament reconstruction was performed in patients with anterior cruciate ligament rupture. Before and after operation, Lysholm score, IKDCand pain visual analogue scale (VAS) were used to evaluate the curative effect of the operation. Objective clinical evaluation was carried out by Barrett standard and single leg long jump test. The result is 1: 1. All patients were followed up for 9 to 29 months, with an average of 19.1 months. According to Barrett criteria, 33 cases of meniscus healing were evaluated. The clinical healing rate was 91.67%. After the last follow-up, the test distance of single leg long jump was 136.67 鹵17.45 cm ~ (-1) in 33 patients with normal limb symmetry. The postoperative Lysholm score and VAS score were significantly higher than those of preoperative Lysholm score (63.72 鹵9.81), postoperatively (86.36 鹵7.91) and IKDC (45.08 鹵6.78), VAS (89.61 鹵4.83), pre-operation (5.19 鹵0.98) and postoperatively (0.75 鹵0.87), respectively. The total excellent and good rate of Lysholm score after operation was 91.7%. Conclusion: arthroscopic outside-in technique for meniscus anterior horn injury repair is simple, without special instruments, less complications, safe and reliable, high clinical healing rate, high satisfaction rate and significant curative effect.
