【摘要】：目的 :探尋影響逆向腋淋巴顯影(axillary reverse mapping,ARM)術(shù)中成功率的因素,了解ARM淋巴結(jié)的解剖分布、保留乳癌患者ARM淋巴結(jié)的風(fēng)險(xiǎn)。方法:對(duì)42例行腋窩淋巴結(jié)清掃的患者進(jìn)行ARM,術(shù)中探尋藍(lán)染淋巴結(jié)及淋巴管,常規(guī)清掃腋窩淋巴組織(包含ARM淋巴結(jié)),記錄ARM操作成功數(shù)目和ARM淋巴結(jié)、淋巴管分布情況,將ARM成功率與患者年齡、身高、體重、上臂圍和腋窩淋巴結(jié)轉(zhuǎn)移情況進(jìn)行相關(guān)性分析,統(tǒng)計(jì)ARM淋巴結(jié)病理結(jié)果,并與患者惡性腫瘤病理分期、免疫組化結(jié)果等進(jìn)行相關(guān)性分析。結(jié)果:26例成功探及藍(lán)染淋巴管和(或)淋巴結(jié),成功率約61.9%,平均探及藍(lán)染淋巴結(jié)1.54枚,ARM成功率與年齡、體重指數(shù)及上臂圍有顯著相關(guān)。送檢的ARM淋巴結(jié)中,2例受腫瘤細(xì)胞侵襲,但ARM淋巴結(jié)被腫瘤侵襲的相關(guān)因素未能明確。結(jié)論:ARM臨床操作成功率受患者個(gè)體自然因素影響,ARM淋巴結(jié)有腫瘤細(xì)胞侵襲的可能,選擇ARM的適應(yīng)證非常重要。
[Abstract]:Objective: to explore the factors influencing the success rate of antiaxillary reverse mapping (ARM) in order to understand the anatomical distribution of arm lymph nodes and the risk of preserving arm lymph nodes in patients with breast cancer. Methods: ARM was performed in 42 patients with axillary lymph node dissection. The blue stained lymph nodes and lymphatic vessels were explored during the operation. The armpit lymphoid tissues (including arm lymph nodes) were routinely dissected. The number of successful arm operations and the distribution of arm lymph nodes and lymphatic vessels were recorded. The relationship between arm success rate and age, height, weight, arm circumference and axillary lymph node metastasis was analyzed, and the pathological results of arm lymph nodes were analyzed. Results the successful rate of blue stained lymphatic vessels and / or lymph nodes was 61.9% in 26 cases. The average success rate of arm in blue stained lymph nodes was significantly correlated with age, body mass index (BMI) and upper arm circumference. Two of the arm lymph nodes were invaded by tumor cells, but the factors related to the invasion of arm lymph nodes were not clear. Conclusion the successful rate of clinical operation of arm may be affected by individual natural factors, and it is very important to select the indication of arm.
