【摘要】：目的探討微創(chuàng)顳淺動脈-大腦中動脈吻合術(shù)(STA-MCA)結(jié)合硬膜翻轉(zhuǎn)術(shù)治療成人缺血型煙霧病的療效。方法回顧性分析2013年3月-2016年3月54例行微創(chuàng)STA-MCA結(jié)合硬膜翻轉(zhuǎn)術(shù)的成人缺血型煙霧病患者的臨床及隨訪資料。結(jié)果行單側(cè)手術(shù)41例,雙側(cè)手術(shù)13例,手術(shù)順利,STA-MCA吻合通暢,患者手術(shù)側(cè)血流灌注均有立即改善;1例患者術(shù)后出現(xiàn)一過性口齒不清,1例術(shù)后出現(xiàn)癥狀性過度灌注,其余患者圍手術(shù)期無并發(fā)癥;隨訪3月~3年,預后良好,DSA示搭橋血管充盈良好,煙霧狀血管有不同程度的減少,側(cè)支循環(huán)建立良好。結(jié)論微創(chuàng)聯(lián)合血管重建術(shù)并發(fā)癥少,手術(shù)療效良好,遠期療效有待于進一步隨訪。
[Abstract]:Objective to investigate the effect of minimally invasive superficial temporal artery-middle cerebral artery anastomosis (STA-MCA) combined with dural reversal in the treatment of adult ischemic moyamoya disease. Methods the clinical and follow-up data of 54 adult patients with ischemic moyamoya disease treated with minimally invasive STA-MCA combined with dural reversal from March 2013 to March 2016 were retrospectively analyzed. Results 41 cases underwent unilateral operation and 13 cases underwent bilateral operation. The anastomosis of STA-MCA was smooth. The perfusion of blood flow on the side of the operation was improved immediately in 1 case, and the symptom overperfusion was found in 1 case after operation. The other patients were followed up for 3 months to 3 years, and the prognosis was good. DSA showed that the blood vessels filled well, the smog vessels decreased in varying degrees, and the collateral circulation was established well. Conclusion minimally invasive combined revascularization has few complications and good curative effect, and the long-term effect should be followed up further.
