后踝骨折的損傷機制及手術(shù)治療結(jié)果分析

【摘要】:[目的]探討后踝骨折的損傷機制,并對33例患者臨床治療效果進行分析。[方法]回顧性分析本院2012年1月~2014年2月收治的33例單側(cè)后踝骨折患者病例,分析患者術(shù)前X線片、CT結(jié)果,進行Danis-Weber分型、Lauge-Hansen分型、Haraguchi分型,并對后踝骨折的損傷機制進行判斷。術(shù)后采用Baird-Jackson踝關節(jié)功能評定標準對手術(shù)療效進行評價。[結(jié)果]根據(jù)患者影像資料,將骨折分型:Danis-Weber:A型0例、B型21例、C型12例;Haraguchi:Ⅰ型14例、Ⅱ型19例、Ⅲ型0例;Lauge-Hansen:旋后外旋19例、旋前外旋3例、旋后內(nèi)收1例、無法分型10例。通過分析,作者認為后踝骨折的損傷機制可能有垂直應力、旋轉(zhuǎn)應力、內(nèi)收應力、混合應力等。術(shù)后隨訪9~14個月,骨折均獲骨性愈合。末次隨訪Baird-Jackson評分:優(yōu)21例、良8例、中4例。[結(jié)論]后踝骨折損傷機制復雜多樣,臨床治療應依據(jù)損傷情況合理選擇手術(shù)入路進行固定治療。
[Abstract]:Objective: to investigate the mechanism of posterior ankle fracture and to analyze the clinical effect of 33 cases. [methods] 33 cases of unilateral posterior malleolar fracture from January 2012 to February 2014 were retrospectively analyzed. The results of X-ray and CT were analyzed before operation, and Haraguchi classification was performed for Danis-Weber classification and Lauge-Hansen classification, and the injury mechanism of posterior malleolus fracture was judged.
