【摘要】：目的探討治療性ERCP技術(shù)在治療老年人膽總管結(jié)石安全性及有效性。方法回顧性分析2015年3月1日至2016年3月1日在廣西醫(yī)科大學內(nèi)鏡中心行內(nèi)鏡逆行胰膽管造影術(shù)(ERCP)治療老年膽胰系統(tǒng)疾病患者的臨床資料,對比老年組和非老年組患者膽石癥ERCP操作種類頻率、治療成功率、并發(fā)癥發(fā)生率、死亡率等。結(jié)果本研究中236名患者共行ERCP取石治療267次,A組為年齡大于80歲患者,共行ERCP操作20次;B組為年齡60歲至80歲患者,共行ERCP操作110次;C組為年齡18歲至60歲的中青年組,共計137次操作。A組、B組、C組患者ERCP取石治療好轉(zhuǎn)出院率分別為100.0%、99.0%、100.0%,完全取石成功率分別為70.6%、75.0%、91.9%。3組患者ERCP治療有效率、術(shù)后胰腺炎、膽管炎、出血等并發(fā)癥發(fā)生率差異無統(tǒng)計學意義,無ERCP所致穿孔發(fā)生,總計ERCP相關(guān)死亡0人。結(jié)論在做好術(shù)前評估及預防情況下,ERCP應(yīng)用于老年人膽總管結(jié)石的治療是安全、有效的。
[Abstract]:Objective to evaluate the efficacy and safety of therapeutic ERCP in the treatment of choledocholithiasis in the elderly. Methods the clinical data of elderly patients with cholangiopancreatic diseases treated by endoscopic retrograde cholangiopancreatography from March 1, 2015 to March 1, 2016 were retrospectively analyzed. The frequency of ERCP operation, the success rate of treatment, the incidence of complications and mortality were compared between the elderly group and the non-elderly group. Results in this study, 236 patients were treated with ERCP lithotomy 267 times. Group A was over 80 years old, group B was aged 60 to 80 years old. Group C was treated with ERCP for 110 times. Group C was a young and middle-aged group aged 18 to 60 years. A total of 137 operations. The rate of improvement of ERCP lithotomy and discharge in group C was 100.00.The success rate of complete lithotomy was 70.6%. The effective rate of ERCP treatment was 75.0%. There was no significant difference in the incidence of postoperative complications such as pancreatitis, cholangitis and hemorrhage. No ERCP related perforation occurred, and a total of 0 ERCP related deaths occurred. Conclusion ERCP is safe and effective in the treatment of choledocholithiasis in the elderly.
