【摘要】：膽囊結(jié)石合并膽總管結(jié)石的傳統(tǒng)治療手段為開(kāi)腹膽囊切除+膽總管探查+T管引流,盡管該方法療效確切,但創(chuàng)傷較大。隨著腔鏡技術(shù)經(jīng)驗(yàn)的累積、微創(chuàng)理念的深入,膽囊結(jié)石合并膽總管結(jié)石的治療方法出現(xiàn)巨大變革,現(xiàn)階段常用的微創(chuàng)治療方法有兩種:腹腔鏡膽囊切除術(shù)聯(lián)合內(nèi)鏡逆行胰膽管造影/內(nèi)鏡下括約肌切開(kāi)取石術(shù)及腹腔鏡聯(lián)合膽道鏡膽總管探查術(shù),根據(jù)內(nèi)鏡治療的時(shí)相、取石途徑、膽管縫合方式等不同,具體操作方法也不同。本文作者對(duì)各種方法的優(yōu)點(diǎn)、缺點(diǎn)、適應(yīng)證和并發(fā)癥作一綜述,為臨床上選擇治療方法提供一定的依據(jù)。
[Abstract]:The traditional treatment of cholecystolithiasis combined with choledocholithiasis was open cholecystectomy and choledochus exploration and T-tube drainage. With the accumulation of experience in endoscopic technique and the deepening of minimally invasive concept, the treatment of cholecystolithiasis with choledocholithiasis has changed greatly. At present, there are two commonly used minimally invasive methods: laparoscopic cholecystectomy combined with endoscopic retrograde cholangiopancreatography / endoscopic sphincterotomy and laparoscopic combined choledochoscopy with common bile duct exploration. Different ways of stone extraction, suture of bile duct, and so on, the specific operation methods are also different. The author reviewed the advantages, disadvantages, indications and complications of various methods, which provided a basis for clinical treatment.
