【摘要】：目的比較交鎖髓內(nèi)釘(IMN)與經(jīng)皮鎖定加壓鋼板(LCP)內(nèi)固定治療脛骨遠(yuǎn)端骨折的臨床療效。方法手術(shù)治療87例脛骨遠(yuǎn)端骨折患者,其中42例行經(jīng)皮LCP固定,45例行IMN固定。比較兩組手術(shù)時(shí)間、術(shù)中出血量、并發(fā)癥、骨折愈合時(shí)間及末次隨訪足踝功能AOFAS評(píng)分。結(jié)果患者均獲得隨訪,時(shí)間12~45個(gè)月。兩組手術(shù)時(shí)間、術(shù)中出血量、骨折愈合時(shí)間、末次隨訪AOFAS評(píng)分比較差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。膝關(guān)節(jié)疼痛、骨折畸形愈合發(fā)生率IMN組高于經(jīng)皮LCP組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。術(shù)后感染發(fā)生率經(jīng)皮LCP組高于IMN組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論 IMN和經(jīng)皮LCP內(nèi)固定治療脛骨遠(yuǎn)端骨折均可取得滿意療效。術(shù)后膝關(guān)節(jié)疼痛、骨折畸形愈合等并發(fā)癥方面經(jīng)皮LCP內(nèi)固定更具有優(yōu)勢(shì),但應(yīng)注意預(yù)防感染。
[Abstract]:Objective to compare the clinical effects of interlocking intramedullary nail (IMN) and percutaneous locking compression plate (LCP) in the treatment of distal tibial fractures. Methods Eighty-seven patients with distal tibial fractures were treated surgically, of which 42 were treated with percutaneous LCP fixation and 45 with IMN fixation. The operative time, intraoperative bleeding, complications, fracture healing time and AOFAS score of ankle function at the last follow-up were compared between the two groups. Results all patients were followed up for 12 ~ 45 months. There were no significant differences in operation time, intraoperative bleeding volume, fracture healing time and AOFAS score between the two groups. The incidence of knee joint pain and fracture malunion in IMN group was higher than that in percutaneous LCP group (P 0.05). The incidence of postoperative infection in LCP group was higher than that in IMN group (P 0.05). Conclusion the treatment of distal tibial fractures with IMN and percutaneous LCP is satisfactory. Postoperative knee joint pain, fracture malunion and other complications such as percutaneous LCP internal fixation is more advantageous, but we should pay attention to prevent infection.
