【摘要】：目的研究益氣活血藥物防治全膝關(guān)節(jié)表面置換(TKA)術(shù)后下肢深靜脈血栓(LDVT)的療效與機(jī)制。方法 60例擬行單側(cè)TKA的膝骨關(guān)節(jié)炎患者,分為觀察組和對(duì)照組各30例,觀察組術(shù)后口服益氣活血湯聯(lián)合靜脈滴注血栓通,對(duì)照組術(shù)后口服利伐沙班。觀察術(shù)后血清D-二聚體、血小板、凝血四項(xiàng)、大腿腫脹率、靜脈彩超等。結(jié)果兩組術(shù)后第10天均無LDVT形成。觀察組術(shù)后第10天的大腿周徑小于對(duì)照組(P=0.021)。觀察組術(shù)后第3天的D-二聚體低于對(duì)照組(P=0.021)。觀察組術(shù)后第10天的血小板水平明顯高于對(duì)照組(P=0.017)。觀察組術(shù)后第1、10天凝血酶時(shí)間均高于對(duì)照組(P0.05)。觀察組術(shù)后第3天纖維蛋白原低于對(duì)照組(P=0.049)。結(jié)論益氣活血中藥可有效防治TKA術(shù)后LDVT,與口服利伐沙班相比,能夠更好地調(diào)節(jié)術(shù)后凝血功能異常,減輕下肢腫脹。
[Abstract]:Objective to study the effect and mechanism of Yiqi Huoxue medicine on the prevention and treatment of deep vein thrombosis (LDVT) after total knee replacement (TKA). Methods 60 patients with unilateral TKA of knee osteoarthritis were divided into observation group and control group, 30 cases in each group. The oral administration of Yiqi Huoxue Decoction Combined with intravenous drip of thrombus after operation, and the control group was taken orally after the operation. The serum D- two polymer, platelets, blood coagulation four, thigh swelling rate and venous color Doppler were observed after operation. Results the two groups had no LDVT formation at tenth days after operation. The thigh diameter of the observation group was less than the control group at tenth days after operation (P=0.021). The D- two body of the observation group was lower than the control group at third days after operation (P=0.021). The platelet level in the observation group was at the tenth day after operation. The time of thrombin in the observation group was higher than the control group (P=0.017) at 1,10 days after operation (P0.05). The fibrinogen of the observation group was lower than the control group (P=0.049) third days after operation (P=0.049). Conclusion Yiqi Huoxue Chinese medicine can effectively prevent and control LDVT after TKA operation. Compared with the oral Lev Shaaban, it can better regulate the abnormal coagulation function and reduce the lower extremity after the operation. Swollen.
