【摘要】：目的探討骨科患者在手術(shù)后發(fā)生感染情況及檢測(cè)白細(xì)胞介素-6(IL-6)、白細(xì)胞介素-8(IL-8)和T淋巴細(xì)胞亞群水平變化的臨床研究,為骨科術(shù)后感染的臨床防治和治療提供參考依據(jù)。方法選取2014年12月-2016年10月期間醫(yī)院進(jìn)行骨科手術(shù)的患者65例為研究對(duì)象,對(duì)患者的術(shù)后感染發(fā)生情況進(jìn)行統(tǒng)計(jì),術(shù)后發(fā)生感染患者21例作為感染組,術(shù)后未發(fā)生感染患者44例設(shè)為對(duì)照組,并將感染患者的致病菌進(jìn)行培養(yǎng)鑒定,同時(shí)觀察患者的IL-6、IL-8及T淋巴細(xì)胞亞群的變化。結(jié)果研究中的65例骨科術(shù)后患者有21例發(fā)生感染,感染率為32.31%;感染患者共計(jì)檢測(cè)病原菌84株,其中革蘭陰性菌51株占60.71%,革蘭陽性菌29株占34.52%,真菌4株占4.76%;感染患者的IL-6(42.16±3.52)ng/L、IL-8(54.15±7.91)ng/L明顯高于非感染患者的IL-6(14.81±7.35)ng/L、IL-8(42.18±10.17)ng/L水平;感染患者的CD_3~+(58.29±6.04)%、CD_4~+(43.94±2.75)%、CD_4~+/CD_8~+(0.64±0.03)明顯高于非感染患者的CD_3~+(49.01±5.72)%、CD_4~+(22.59±3.27)%、CD_4~+/CD_8~+(1.08±0.31)。結(jié)論骨科患者在術(shù)后發(fā)生感染的情況較為嚴(yán)重,其中革蘭陰性菌感染居多,感染患者的IL-6、IL-8與T淋巴細(xì)胞亞群水平較非感染患者明顯升高,提示可作為臨床判斷術(shù)后感染的重要指標(biāo)。
[Abstract]:Objective to investigate the incidence of infection in orthopedic patients after surgery and to investigate the changes of interleukin-6 (IL-6), interleukin-8 (IL-8) and T lymphocyte subsets in orthopedic patients, and to provide a reference for the prevention and treatment of postoperative infection in orthopedic patients. Methods Sixty-five patients undergoing orthopedic surgery in hospital from December 2014 to October 2016 were selected as subjects. The incidence of postoperative infection was statistically analyzed. 21 patients with postoperative infection were selected as infection group. Forty-four patients without infection after operation were set up as control group. The pathogenic bacteria of infected patients were cultured and identified. The changes of IL-6 IL-8 and T-lymphocyte subsets were observed at the same time.
