【摘要】：目的對人工全膝關(guān)節(jié)置換術(shù)(total knee arthroplasty,TKA)中屈曲間隙大于伸直間隙的相關(guān)研究進(jìn)展進(jìn)行綜述。方法廣泛查閱國內(nèi)外有關(guān)膝關(guān)節(jié)屈伸間隙影響因素,以及TKA術(shù)中增加屈曲間隙后膝關(guān)節(jié)生物力學(xué)及運(yùn)動學(xué)改變特點(diǎn)、臨床效果等方面的研究文獻(xiàn),并進(jìn)行總結(jié)分析。結(jié)果 TKA術(shù)中調(diào)整屈伸間隙關(guān)系是影響手術(shù)成敗關(guān)鍵因素之一。目前大量生物力學(xué)、運(yùn)動學(xué)及臨床研究顯示,TKA術(shù)中屈曲間隙適當(dāng)大于伸直間隙可以改善術(shù)后膝關(guān)節(jié)活動度、提高患者滿意度,同時不影響膝關(guān)節(jié)穩(wěn)定性;但也有研究結(jié)果與之相反。因此TKA術(shù)中對于屈伸間隙的調(diào)整尚存在爭議。結(jié)論 TKA術(shù)中調(diào)整屈曲間隙至適當(dāng)大于伸直間隙是一種新的關(guān)節(jié)間隙理論,其中遠(yuǎn)期臨床效果、術(shù)中操作技巧以及相關(guān)并發(fā)癥有待后續(xù)臨床實(shí)踐探索。
[Abstract]:Objective to review the research progress of flexion space larger than extension space in total knee arthroplasty. Methods domestic and foreign literatures on the influencing factors of flexion and extension space of knee joint, biomechanical and kinematic changes of knee joint after TKA operation and clinical effect were reviewed and analyzed. Results adjusting the relationship between flexion and extension space during TKA was one of the key factors affecting the success or failure of the operation. At present, a large number of biomechanics, kinematics and clinical studies show that the flexion space is larger than the extension space in TKA operation can improve the knee motion, improve patient satisfaction, while not affect the stability of the knee. But there are also studies to the contrary. Therefore, the adjustment of flexion and extension space during TKA is still controversial. Conclusion it is a new theory of joint space to adjust the flexion space to be larger than the extension space in TKA. The long-term clinical effect, operative skills and related complications need to be explored in the follow-up clinical practice.
