【摘要】：目的探討帶線錨釘與微鈦板固定對(duì)頸椎單開門椎管擴(kuò)大成形術(shù)后門軸側(cè)椎板骨愈合的影響。方法回顧性研究采用頸椎單開門術(shù)治療的脊髓型頸椎病56例,根據(jù)內(nèi)固定方式分為錨釘組和微鈦板組,其中錨釘組21例,共開門105個(gè)椎板,微鈦板組35例,共開門175個(gè)椎板。評(píng)估2組術(shù)后臨床癥狀、神經(jīng)功能和頸椎活動(dòng)度的改變。根據(jù)術(shù)后3 d CT評(píng)估術(shù)后門軸椎板的骨折情況,分為門軸骨折移位組和門軸骨折無移位組;根據(jù)術(shù)后3個(gè)月和6個(gè)月的CT評(píng)估門軸的骨愈合情況。結(jié)果 2組術(shù)后3個(gè)月和6個(gè)月JOA評(píng)分較術(shù)前明顯提高(P均0.05),而2組間JOA評(píng)分及神經(jīng)功能改善率比較差異無統(tǒng)計(jì)學(xué)意義(P均0.05)。微鈦板組術(shù)后6個(gè)月ROM及VAS評(píng)分優(yōu)于錨釘組(P均0.05)。微鈦板組術(shù)后3個(gè)月總體骨愈合率高于錨釘組(P0.05),2組術(shù)后6個(gè)月總體骨愈合率比較差異無統(tǒng)計(jì)學(xué)意義(P0.05);微鈦板組門軸骨折無移位亞組術(shù)后3個(gè)月骨愈合率高于錨釘組(P0.05),術(shù)后6個(gè)月2組差異無統(tǒng)計(jì)學(xué)意義(P0.05),微鈦板組骨折移位亞組術(shù)后3個(gè)月及術(shù)后6個(gè)月骨愈率均高于錨釘組(P均0.05)。結(jié)論相對(duì)于帶線錨釘固定法,微鈦板固定法對(duì)單開門術(shù)后門軸的穩(wěn)定性和骨愈合作用更佳,兩者在神經(jīng)功能改善方面無差異。
[Abstract]:Objective to investigate the effect of wire anchor nail and microtitanium plate fixation on the union of axial laminar bone in cervical open door laminoplasty. Methods 56 cases of cervical Spondylotic myelopathy treated by single open door cervical vertebra were retrospectively studied. According to the internal fixation, 56 cases were divided into two groups: the anchor nail group (21 cases), the open door laminae group (35 cases) and the microtitanium plate group (35 cases). To evaluate the changes of clinical symptoms, neurological function and cervical mobility after operation in both groups. According to the CT data of 3 days after operation, the patients were divided into two groups: the displaced group and the non-displaced group, and the bone union of the portal axis was evaluated by CT at 3 and 6 months after operation. Results the JOA scores of the two groups increased significantly 3 and 6 months after operation (P < 0.05), but there was no significant difference between the two groups in the scores of JOA and the improvement rate of nerve function (P < 0.05). The ROM and VAS scores in the microtitanium plate group were better than those in the anchor nail group at 6 months after operation (P < 0.05). There was no significant difference in the total bone healing rate between the microtitanium plate group and the anchor nail group at 3 months postoperatively, and the total bone healing rate in the titanium microplate group was higher than that in the anchor nail group at 3 months after operation, and the total bone union rate in the titanium microplate group was higher than that in the anchor nail group at 3 months after operation. There was no significant difference between the two groups at 6 months after operation (P 0.05). The recovery rate of bone in the microplate group was higher than that in the anchor nail group at 3 months after operation and 6 months after operation (P < 0.05). Conclusion compared with wire anchor nail fixation, microtitanium plate fixation has better effect on stability and bone healing of portal axis after single door opening operation, and there is no difference in the improvement of nerve function between the two methods.
