【摘要】：目的評(píng)估超聲引導(dǎo)下真空輔助旋切對(duì)乳腺良性疾病手術(shù)切除的臨床應(yīng)用價(jià)值。方法回顧性研究2009年1月~2014年1月在貴陽(yáng)市婦幼保健院乳腺科進(jìn)行乳腺微創(chuàng)旋切手術(shù)的1267名乳腺良性疾病患者在切除率、并發(fā)癥發(fā)生率、復(fù)發(fā)率及滿意度方面臨床隨訪結(jié)果及對(duì)相關(guān)危險(xiǎn)因素分析。術(shù)后于1月、6月及其后每6月進(jìn)行隨訪至2年(中位隨訪22個(gè)月)。結(jié)果(1)1267例中1259例完全切除,8例術(shù)后1月發(fā)現(xiàn)殘留,手術(shù)切除率99.36%。手術(shù)切除率與病灶大小相關(guān)(P=0.003),而與年齡、BI-RADS分級(jí)、病灶數(shù)目及位置無(wú)明顯相關(guān)(P值均0.05)。(2)1267例中89例出現(xiàn)并發(fā)癥(7.02%),其中血腫70例(5.52%),發(fā)生率與病灶數(shù)目及位置相關(guān)(P=0.000),而與年齡、BI-RADS分級(jí)、病灶大小無(wú)明顯相關(guān)(P值均0.05)。(3)1267例中193例患者被確診有復(fù)發(fā),復(fù)發(fā)率15.23%,其中原位復(fù)發(fā)65例(5.13%),新發(fā)病變128例(10.1%)。復(fù)發(fā)率與病灶數(shù)目及大小相關(guān)(P=0.000),而與年齡、BI-RADS分級(jí)、病灶位置無(wú)明顯相關(guān)(P值均0.05)。(4)1267例患者有6例對(duì)手術(shù)外觀不滿意,患者滿意度達(dá)99.52%。結(jié)論超聲引導(dǎo)下MMT真空輔助旋切對(duì)乳腺良性疾病是一種安全有效的手術(shù),手術(shù)并發(fā)癥少,切除率高,復(fù)發(fā)率低,對(duì)良性和高風(fēng)險(xiǎn)乳腺病變,特別是多發(fā)性乳房良性病變具有滿意的外觀效果。
[Abstract]:Objective to evaluate the clinical value of ultrasound-guided vacuum-assisted excision for benign breast diseases. Methods from January 2009 to January 2014, 1267 patients with benign breast diseases underwent minimally invasive mastectomy from January 2009 to January 2014 in the Department of Breast of Guiyang Maternal and Child Health Hospital. The clinical follow-up results and risk factors were analyzed in the aspects of recurrence rate and satisfaction. The patients were followed up to 2 years in 1 month, 6 months and every 6 months thereafter (median 22 months follow-up). Results 1259 cases were completely resected and 8 cases were found residual one month after operation, the resectability rate was 99.36%. The surgical resection rate was correlated with the size of the lesion. However, there was no significant correlation between the surgical resection rate and the BI-RADS grade, the number and location of the lesions, the number and location of the lesions (P = 0.05). The complications were found in 89 of the 1267 cases. The incidence of hematoma was 5.52%, and the incidence was correlated with the number and location of the lesions (P0.000), but with the age BI-RADS grade. There was no significant correlation between the size of lesions and P values of 193 cases of 1267 patients. The recurrence rate was 15.23. 65 cases were in situ recurrence, and 128 cases were new lesions with a recurrence rate of 10. 1% (P < 0. 05, P = 0. 05, P = 0. 05, P = 0. 05, P = 0. 05, P = 0. 05, P = 0. 05). The recurrence rate was correlated with the number and size of the lesion, but not with the BI-RADS grade. There was no significant correlation between the location of the lesion and the location of the lesion. Six of the 1267 patients were not satisfied with the appearance of the operation, and the patients' satisfaction was 99.52%. Conclusion Ultrasound-guided MMT vacuum assisted rotary resection is a safe and effective procedure for benign breast diseases with less complications, higher resection rate and lower recurrence rate. In particular, multiple benign breast lesions have a satisfactory external effect.
