【摘要】：目的:膝關(guān)節(jié)骨性關(guān)節(jié)炎是一種常見(jiàn)的慢性關(guān)節(jié)疾病,好發(fā)于中老年人,嚴(yán)重影響中老年人的身體健康及生活質(zhì)量。本次研究通過(guò)臨床觀察中藥塌漬聯(lián)合關(guān)節(jié)腔注射玻璃酸鈉治療風(fēng)寒濕痹型膝關(guān)節(jié)骨性關(guān)節(jié)炎的臨床療效,試圖為臨床治療膝關(guān)節(jié)骨性關(guān)節(jié)炎提供一種簡(jiǎn)單而行之有效的中西醫(yī)結(jié)合治療方法提供依據(jù)。方法:本研究所采用病例均來(lái)自長(zhǎng)春中醫(yī)藥大學(xué)附屬醫(yī)院骨科住院部患者,共60例。住院期間為2013年11月至2015年1月,且均符合相應(yīng)的膝骨性關(guān)節(jié)炎診斷納入標(biāo)準(zhǔn)及相關(guān)標(biāo)準(zhǔn);納入后整體采用隨機(jī)分組原則,分為治療組與對(duì)照組,兩組各30例。治療組給予外敷中藥塌漬并關(guān)節(jié)腔內(nèi)注射玻璃酸鈉治療;對(duì)照組給予外用貼覆精致狗皮膏并關(guān)節(jié)腔內(nèi)注射玻璃酸鈉治療。經(jīng)過(guò)2個(gè)療程1個(gè)觀察周期后,對(duì)各組的治療情況與膝關(guān)節(jié)改善情況分別進(jìn)行匯總與評(píng)分,以評(píng)價(jià)總體療效。根據(jù)膝關(guān)節(jié)骨性關(guān)節(jié)炎疾病特點(diǎn)及患者自身情況,本研究評(píng)定標(biāo)準(zhǔn)采用2002年《中藥新藥臨床研究指導(dǎo)原則》的中醫(yī)證候評(píng)分標(biāo)準(zhǔn)、1976年美國(guó)特種外科醫(yī)院(HSS)的膝關(guān)節(jié)功能評(píng)分標(biāo)準(zhǔn)、視覺(jué)模擬評(píng)分表(VAS)分別給予評(píng)分,并將匯總后所有數(shù)據(jù)經(jīng)spssl7.0統(tǒng)計(jì)軟件進(jìn)行結(jié)果分析,以便客觀評(píng)價(jià)治療后兩種療法的總體療效。結(jié)果:治療組與對(duì)照組治療后在中醫(yī)證候改善方面比較分析可見(jiàn)(P0.05),說(shuō)明治療組在中醫(yī)證候改善方面療效好于對(duì)照組;另外根據(jù)(HSS)的膝關(guān)節(jié)功能評(píng)分標(biāo)準(zhǔn)在疼痛、關(guān)節(jié)功能改善、關(guān)節(jié)活動(dòng)度等方面(P0.05),表明治療組療效也較對(duì)照組有優(yōu)勢(shì);最后進(jìn)行整體療效對(duì)比經(jīng)秩和檢驗(yàn)Z=-2.981,P0.05,表示兩組之間差異有統(tǒng)計(jì)學(xué)意義,結(jié)果說(shuō)明治療組整體療效明顯優(yōu)于對(duì)照組。結(jié)論:經(jīng)臨床研究比較得出中藥溻漬聯(lián)合關(guān)節(jié)腔注射玻璃酸鈉療法在治療膝關(guān)節(jié)骨性關(guān)節(jié)炎方面療效顯著,效果獨(dú)到,為中西醫(yī)結(jié)合療法治療膝骨性關(guān)節(jié)炎提供了新思路,也為后續(xù)開(kāi)發(fā)高效、價(jià)廉、操作簡(jiǎn)便的治療方法提供了理論依據(jù)。
[Abstract]:Objective: osteoarthritis of knee joint is a common chronic joint disease. In this study, we observed the clinical curative effect of the treatment of rheumatism and dampness arthralgia type osteoarthritis of knee joint with traditional Chinese medicine cavities and intraarticular injection of sodium hyaluronate. This paper attempts to provide a simple and effective method for the treatment of knee osteoarthritis. Methods: 60 cases of orthopedic patients from affiliated Hospital of Changchun University of traditional Chinese Medicine were enrolled in this study. The period of hospitalization was from November 2013 to January 2015, and all of them met the relevant criteria for the diagnosis of osteoarthritis of the knee, and were divided into treatment group and control group with 30 cases in each group. The treatment group was treated with external application of traditional Chinese medicine and intraarticular injection of sodium hyaluronate, while the control group was treated with external application of refined canine skin ointment and intraarticular injection of sodium hyaluronate. After two courses of treatment and one observation cycle, the treatment and knee joint improvement in each group were summarized and scored respectively to evaluate the overall curative effect. According to the characteristics of knee osteoarthritis disease and the patient's own condition, The evaluation criteria of this study were based on the criteria of TCM syndromes score of 2002 "guiding principles of Clinical Research of New drugs of traditional Chinese Medicine", the knee joint function score of American Special surgery Hospital (HSS) in 1976 and visual analogue scale (VAS), respectively. All the data were analyzed by spssl7.0 software in order to evaluate the overall curative effect of the two treatments objectively. Results: the improvement of TCM syndromes in the treatment group and the control group was compared with that in the control group, indicating that the curative effect of the treatment group was better than that of the control group in the improvement of TCM syndromes, in addition, according to the score of knee joint function of HSS, pain and joint function were improved. The results showed that the therapeutic effect of the treatment group was better than that of the control group, and the overall curative effect of the treatment group was better than that of the control group by rank sum test. Conclusion: compared with the clinical study, it is concluded that the treatment of traditional Chinese medicine Tachia and intraarticular injection of sodium hyaluronate is effective and unique in the treatment of knee osteoarthritis, which provides a new idea for the treatment of knee osteoarthritis by the combination of traditional Chinese and Western medicine. It also provides a theoretical basis for the further development of effective, inexpensive and simple treatment methods.
