【摘要】：目的：目前大部分研究以TKA后關(guān)節(jié)活動(dòng)功能和膝關(guān)節(jié)假體長(zhǎng)期存活率作為判斷全膝關(guān)節(jié)置換術(shù)(TKA)是否成功的主要標(biāo)準(zhǔn),而尚無(wú)有關(guān)TKA對(duì)患者維持軀體平衡性能力影響的研究報(bào)道。本研究中探討TKA對(duì)膝骨關(guān)節(jié)炎(0A)患者軀體靜態(tài)平衡性(body balance)的影響,并分析患者軀體靜態(tài)平衡性的變化與術(shù)后膝關(guān)節(jié)功能恢復(fù)情況是否存在相關(guān)性。方法：對(duì)2011年1月至2013年12月在我院骨科中心關(guān)節(jié)外科行單側(cè)TKA的膝0A患者21例(右側(cè)12例,左側(cè)9例)進(jìn)行站立平衡試驗(yàn),在單位時(shí)間內(nèi)用重心測(cè)量?jī)x連續(xù)測(cè)量軀體重心位置(GCP)。對(duì)GCP在單位時(shí)間內(nèi)的橫軸方向偏移(ML-X range)、縱軸方向偏移(AP-Y range)以及GCP漂移軌跡(LG)進(jìn)行量化分析,探討GCP的ML-X range、AP-Y range以及LG變化程度與TKA后患者膝關(guān)節(jié)恢復(fù)情況之間的相關(guān)性。結(jié)果：兩組患者經(jīng)TKA后GCP在橫軸方向上的平均位置由初始位置逐漸向術(shù)側(cè)方向轉(zhuǎn)移,且其較術(shù)前時(shí)的轉(zhuǎn)移程度和患者LG較術(shù)前時(shí)變化程度均與同一時(shí)期膝關(guān)節(jié)功能較術(shù)前時(shí)水平恢復(fù)情況呈相關(guān)性,而GCP在縱軸方向上的轉(zhuǎn)移與膝關(guān)節(jié)功能恢復(fù)情況無(wú)明顯相關(guān)性。結(jié)論：患者行單側(cè)TKA后,維持軀體平衡能力逐漸提高,且與膝關(guān)節(jié)功能恢復(fù)情況隨術(shù)后時(shí)間呈線性相關(guān)。
[Abstract]:Objective: to evaluate the success of total knee arthroplasty (TKA) by using TKA and long-term survival rate of knee prosthesis as the main criteria. However, there is no study on the effect of TKA on the ability of maintaining somatic balance. The purpose of this study was to investigate the effect of TKA on the body balance in patients with knee osteoarthritis (Osteoarthritis), and to analyze the correlation between the changes of somatic static balance and the recovery of knee joint function after operation. Methods: from January 2011 to December 2013, 21 cases (12 right and 9 left) of knee 0A treated with unilateral TKA in our orthopedic central joint surgery department were tested by standing balance test. The position of body center of gravity was continuously measured with a barycenter meter in unit time. The horizontal axis shift (ML-X range), longitudinal axis shift (AP-Y range) and GCP drift trajectory (LGG) of GCP in unit time were quantitatively analyzed, and the correlation between the ML-X range of GCP and the degree of LG change and the recovery of knee joint after TKA was discussed. Results: after TKA, the average position of GCP in the axial direction gradually shifted from the initial position to the operative side. The degree of metastasis and the change of LG were correlated with the recovery of knee joint function in the same period, but there was no significant correlation between the transfer of GCP in the axial direction and the recovery of knee joint function. Conclusion: after unilateral TKA, the ability of maintaining somatic balance was gradually improved, and the recovery of knee joint function was linearly correlated with postoperative time.
