【摘要】：目的探討不同截骨手術(shù)方案治療內(nèi)翻型膝關(guān)節(jié)炎的近期療效。方法選取內(nèi)翻型膝關(guān)節(jié)炎患者60例,隨機分為觀察組和對照組各30例,觀察組采用腓骨近端截骨術(shù)方案治療,對照組采用脛骨高位截骨術(shù)方案治療。比較兩組患者住院時間、出血量、手術(shù)時間和并發(fā)癥情況等;分別在患者術(shù)前和術(shù)后3、6個月采用美國特種外科醫(yī)院膝關(guān)節(jié)評分(HSS)和美國膝關(guān)節(jié)協(xié)會評分(KSS)標(biāo)準(zhǔn)對患者膝關(guān)節(jié)功能恢復(fù)程度進行判定;術(shù)后12、24、72h采用疼痛視覺模擬評分(VAS)評估膝關(guān)節(jié)前內(nèi)側(cè)不同狀態(tài)下的疼痛程度。結(jié)果觀察組手術(shù)耗時、住院時間均短于對照組,出血量低于對照組(P均0.05);兩組術(shù)后3、6個月不同時間段HSS、KSS無統(tǒng)計學(xué)差異(P均0.05);與術(shù)前相比,術(shù)后3、6個月HSS、KSS均提高(P均0.05);與對照組相比,觀察組術(shù)后不同時間段靜息狀態(tài)、運動狀態(tài)下VAS逐漸降低(P均0.05)。結(jié)論腓骨近端截骨術(shù)治療內(nèi)翻型膝關(guān)節(jié)炎療效與脛骨高位截骨術(shù)相當(dāng);但腓骨近端截骨術(shù)安全性更高,且術(shù)后疼痛感降低,更具有臨床治療優(yōu)勢。
[Abstract]:Objective to explore the short-term effect of different osteotomy for the treatment of varus knee arthritis. Methods 60 cases of varus knee arthritis were selected and divided into the observation group and the control group (30 cases each). The observation group was treated with peroneal proximal osteotomy, and the control group was treated with high tibial osteotomy. The time of hospitalization was compared between the two groups. Blood volume, operation time, and complications were determined before and after 3,6 months of the patient's knee joint score (HSS) and American Knee association score (KSS), respectively, and the postoperative 12,24,72h was evaluated by pain visual analogue score (VAS) for the assessment of the medial knee joint. Results in the same state of pain, the time of operation in the observation group was shorter than that in the control group and the amount of bleeding was lower than that of the control group (P 0.05). There was no statistical difference between the two groups at different time periods HSS and KSS (P 0.05) in the two months after the operation (P 0.05), and 3,6 months after the operation (P 0.05). Compared with the control group, the group was compared with the control group at different time after the operation. In the resting state, VAS decreased gradually (P 0.05). Conclusion proximal peroneal osteotomy for the treatment of varus knee osteoarthritis is equivalent to that of high tibial osteotomy, but the proximal peroneal osteotomy is more safe and less painful after operation, and has a better clinical therapeutic advantage.
