【摘要】：目的:探討腹腔鏡膽囊切除術(shù)聯(lián)合清胰湯治療早期膽源性胰腺炎的臨床效果分析。方法:選取2011年3月—2016年3月本院收治的早期膽源性胰腺炎患者102例,隨機(jī)分為觀察組與對(duì)照組,對(duì)照組患者單純給予腹腔鏡膽囊切除術(shù)治療,觀察組給予腹腔鏡膽囊切除術(shù)聯(lián)合清胰湯治療,治療結(jié)束后對(duì)兩組患者的臨床療效進(jìn)行評(píng)價(jià)對(duì)比,并對(duì)兩組患者實(shí)驗(yàn)室指標(biāo)恢復(fù)時(shí)間、臨床癥狀恢復(fù)時(shí)間、平均住院時(shí)間及治療前后腫瘤壞死因子-α(TNF-α)、C-反應(yīng)蛋白(CRP)水平進(jìn)行對(duì)比。結(jié)果:觀察組患者的總有效率明顯高于對(duì)照組(P0.05);觀察組患者的尿淀粉酶、血淀粉酶、白細(xì)胞恢復(fù)時(shí)間及平均住院時(shí)間均明顯較對(duì)照組縮短(P0.05);觀察組患者體溫恢復(fù)時(shí)間、腹痛消失時(shí)間、腹脹消失時(shí)間、自助排便恢復(fù)時(shí)間、腎功能恢復(fù)時(shí)間均明顯較對(duì)照組縮短(P0.05);治療后,觀察組與對(duì)照組患者的TNF-α與CRP水平均較治療前降低(P0.05),且觀察組患者的TNF-α與CRP水平較對(duì)照組降低的更顯著(P0.05)。結(jié)論:腹腔鏡膽囊切除術(shù)聯(lián)合清胰湯治療早期膽源性胰腺炎具有顯著的療效,可有效縮短治療時(shí)間,值得臨床推廣。
[Abstract]:Objective: to investigate the clinical effect of laparoscopic cholecystectomy combined with Qingyi decoction in the treatment of early biliary pancreatitis. Methods: 102 patients with early biliary pancreatitis from March 2011 to March 2016 were randomly divided into two groups: the observation group and the control group. The patients in the control group were treated with laparoscopic cholecystectomy. The observation group was treated with laparoscopic cholecystectomy combined with Qingyi decoction. After the treatment, the clinical efficacy of the two groups was evaluated and compared, and the recovery time of laboratory indexes and clinical symptoms were compared between the two groups. The mean hospitalization time and the level of tumor necrosis factor-偽-TNF- 偽-TNF- 偽-C- reactive protein (CRP) before and after treatment were compared. Results: the total effective rate of the patients in the observation group was significantly higher than that in the control group (P 0.05), the recovery time of urine amylase, serum amylase, leukocyte and average hospitalization time of the patients in the observation group were significantly shorter than those in the control group, and the recovery time of body temperature in the observation group was significantly shorter than that in the control group. The time of disappearance of abdominal pain, distention, self-help defecation and renal function were significantly shorter than that of control group (P 0.05). The levels of TNF- 偽 and CRP in the observation group and the control group were lower than those before treatment, and the levels of TNF- 偽 and CRP in the observation group were significantly lower than those in the control group. Conclusion: laparoscopic cholecystectomy combined with Qingyi decoction is effective in the treatment of early biliary pancreatitis.
