【摘要】：目的:比較支撐鋼板與鎖定鋼板治療復(fù)雜脛骨平臺(tái)骨折的療效。方法:選取在我院治療的復(fù)雜脛骨平臺(tái)骨折患者45例,時(shí)間為2011年9月~2014年2月,其中25例行雙側(cè)入路鎖定鋼板治療,為A組,雙側(cè)支撐鋼板內(nèi)固定治療20例,為B組。對(duì)比兩組的術(shù)中出血量、手術(shù)時(shí)間、完全負(fù)重時(shí)間有無(wú)區(qū)別,最后一次隨訪應(yīng)用Rasmussen評(píng)分標(biāo)準(zhǔn)對(duì)膝關(guān)節(jié)功能情況進(jìn)行評(píng)估來(lái)比較兩組的膝關(guān)節(jié)優(yōu)良率。結(jié)果:術(shù)后定期對(duì)患者進(jìn)行隨訪,1例失訪,兩組患者骨折均正常愈合,沒(méi)有發(fā)生骨折延期愈合、不愈合情況;A組患者的手術(shù)時(shí)間(138.8±2.9)min、術(shù)中出血量(313.9±3.9)ml,B組患者的手術(shù)時(shí)間(136.7±3.3)min、術(shù)中出血量(317.1±3.8)ml,差異無(wú)有統(tǒng)計(jì)學(xué)意義(P0.05);但A組患者的完全負(fù)重時(shí)間(3.8±0.2)月明顯短于B組的完全負(fù)重時(shí)間(4.7±0.3)月,A組膝關(guān)節(jié)功能優(yōu)良率88%高于B組的70%,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:鎖定鋼板組更符合生物力學(xué)原則,完全負(fù)重時(shí)間明顯早于支撐鋼板組,有利于早期膝關(guān)節(jié)功能鍛煉,膝關(guān)節(jié)優(yōu)良率也高于支撐鋼板組。相對(duì)于支撐鋼板,鎖定鋼板在早期下地鍛煉、改善膝關(guān)節(jié)功能方面有明顯優(yōu)勢(shì)。
[Abstract]:Objective: to compare the curative effect of supporting plate and locking plate in the treatment of complex tibial plateau fracture. Methods: Forty-five patients with complex tibial plateau fractures treated in our hospital from September 2011 to February 2014 were selected. 25 cases were treated with locking plate with bilateral approach, group A and 20 cases with bilateral braced plate internal fixation. The amount of blood loss, time of operation and time of full weight bearing were compared between the two groups. The excellent and good rate of knee joint in the two groups was evaluated by using Rasmussen score in the last follow-up. Results: one patient was followed up regularly after operation. The fracture healed normally in both groups, and there was no delayed union. The operative time was 138.8 鹵2.9 min in group A, 136.7 鹵3.3 min in group B and 317.1 鹵3.8 ml in group B, but the time of complete weight loading in group A was significantly shorter than that in group B (3.8 鹵0.2) months. The excellent and good rate of knee joint function in group A was 88% higher than that in group B (4.7 鹵0.3) months, and the difference was statistically significant (P 0.05). Conclusion: locking plate group is more in line with biomechanical principle, the time of full weight loading is earlier than that of support plate group, which is beneficial to early knee joint function exercise, and the excellent and good rate of knee joint is higher than that of support plate group. Compared with supporting plate, locking plate has obvious advantages in early exercise and improvement of knee joint function.
