【摘要】：[目的]總結(jié)Ilizarov骨延長(zhǎng)術(shù)治療尺橈骨短縮并腕關(guān)節(jié)畸形的臨床經(jīng)驗(yàn),探討該方法的臨床療效及注意事項(xiàng)。[方法]2011年5月~2016年7月,對(duì)12例尺橈骨短縮并腕關(guān)節(jié)畸形患者,應(yīng)用Ilizarov環(huán)形外固定架固定結(jié)合尺骨或橈骨截骨,矯正彎曲畸形及關(guān)節(jié)脫位,同期行延長(zhǎng)術(shù)進(jìn)行治療。[結(jié)果]所有患者尺橈骨彎曲、短縮及腕關(guān)節(jié)畸形均得到矯正,術(shù)后隨訪6~52個(gè)月,延長(zhǎng)4~6.10 cm,平均5 cm。橈尺遠(yuǎn)側(cè)關(guān)節(jié)穩(wěn)定,截骨部位愈合良好,腕關(guān)節(jié)屈曲、背伸活動(dòng)良好,旋轉(zhuǎn)功能改善,手指活動(dòng)良好,無不良并發(fā)癥。[結(jié)論]Ilizarov骨延長(zhǎng)術(shù)治療尺橈骨短縮并腕關(guān)節(jié)畸形能夠改善外觀及功能,損傷小,并發(fā)癥少,是一種較為可靠有效的方法。
[Abstract]:Objective: to summarize the clinical experience of Ilizarov bone lengthening in the treatment of ulnar and radial shortening with wrist joint malformation, and to discuss the clinical effect and matters needing attention. [methods] from May 2011 to July 2016, 12 patients with ulnar and radial shortening combined with wrist joint deformity were treated with Ilizarov ring external fixator combined with ulna or radial osteotomy to correct bending deformity and dislocation of joint. [results] all the patients were corrected for ulnar and radial bone bending, short contraction and wrist joint malformation. The postoperative follow-up was 6 ~ 52 months, and the lengthening was 4 ~ 6.10 cm, with an average of 5 cm. The distal radioulnar joint was stable, the osteotomy site healed well, the wrist joint flexion, the dorsal extension activity was good, the rotation function was improved, the finger movement was good, there was no bad complication. [conclusion] Ilizarov bone lengthening is a reliable and effective method for the treatment of ulnar and radial shortening with wrist deformity.
