個(gè)體化導(dǎo)航模板輔助頸椎椎弓根螺釘置入與徒手螺釘置入的準(zhǔn)確性比較

【摘要】:目的比較個(gè)體化導(dǎo)航模板輔助頸椎椎弓根螺釘置入與徒手螺釘置入的準(zhǔn)確性。方法選取頸椎完整的成人尸體8具,隨機(jī)分為導(dǎo)航輔助組和徒手置釘組各4具。導(dǎo)航輔助組對(duì)尸體頸椎(C3~7)進(jìn)行CT掃描,重建頸椎三維模型,設(shè)計(jì)與頸椎椎板吻合的反向模板,構(gòu)建個(gè)體化導(dǎo)航模板的數(shù)據(jù)模型,采用快速成型技術(shù)打印個(gè)體化導(dǎo)航模板;將導(dǎo)航模板與頸椎椎體后部緊密貼合,輔助置入頸椎椎弓根螺釘。徒手置釘組采用Abumi椎弓根入口直接暴露法進(jìn)行徒手螺釘置入。術(shù)后即刻行頸椎X線和CT斷層掃描,觀察螺釘置入情況,評(píng)價(jià)置釘效果,計(jì)算置釘準(zhǔn)確率。結(jié)果導(dǎo)航輔助組共置入螺釘32枚,置釘效果為Ⅰ級(jí)31枚、Ⅱ級(jí)1枚,置釘準(zhǔn)確率為96.8%(31/32);徒手置釘組共置入螺釘32枚,置釘效果為Ⅰ級(jí)29枚、Ⅱ級(jí)3枚,置釘準(zhǔn)確率為90.6%(29/32);導(dǎo)航輔助組置釘準(zhǔn)確率高于徒手置釘組(P0.05)。結(jié)論個(gè)體化導(dǎo)航模板輔助頸椎椎弓根螺釘置入的準(zhǔn)確性高于徒手螺釘置入。
[Abstract]:Objective to compare the accuracy of pedicle screw placement with individual navigation template in cervical pedicle screw placement. Methods 8 adult cadavers with intact cervical vertebrae were randomly divided into navigation aid group and unarmed nail group. In the navigation assistant group, CT scanning was performed on the cadaveric cervical spine, the three-dimensional model of the cervical vertebra was reconstructed, the reverse template was designed and the data model of the individualized navigation template was constructed, and the individualized navigation template was printed by rapid prototyping. The navigation template is tightly attached to the posterior part of the cervical vertebrae to assist the placement of the cervical pedicle screw. Direct exposure of pedicle entrance of Abumi was used in the naked screw placement group. X-ray and CT scans of cervical spine were performed immediately after operation, screw placement was observed, the effect of screw placement was evaluated, and the accuracy of nail placement was calculated.
