【摘要】：目的評估自體脂肪源性干細(xì)胞(ADSCs)聯(lián)合無菌生物護(hù)創(chuàng)膜對慢性創(chuàng)面愈合的作用。方法選取武警總醫(yī)院收治的60例慢性創(chuàng)面患者,隨機(jī)分為凡士林油紗組(A組)、無菌生物護(hù)創(chuàng)膜組(B組)、ADSCs聯(lián)合無菌生物護(hù)創(chuàng)膜組(C組),每組20例。觀察3組創(chuàng)面愈合情況、愈合時間及治療后7、21、40d時的創(chuàng)面愈合率,治療前、治療7d、治療14d時創(chuàng)面上皮基底層細(xì)胞增殖情況及治療后50d創(chuàng)面上皮化情況,計數(shù)治療前和治療后7d創(chuàng)面上皮基底層新生微小血管密度(MVD)。結(jié)果與A、B組創(chuàng)面愈合情況比較,C組創(chuàng)周炎性反應(yīng)輕,肉芽組織形成好,創(chuàng)緣上皮爬行速度快,且B組創(chuàng)面愈合情況好于A組。創(chuàng)面愈合時間A組B組C組,組間比較差異有統(tǒng)計學(xué)意義(P0.05)。3組治療后各時間點創(chuàng)面愈合率、創(chuàng)面上皮基底層增殖細(xì)胞數(shù)、新生微小血管密度比較均為C組優(yōu)于B組優(yōu)于A組,差異有統(tǒng)計學(xué)意義(P0.05),治療后50d創(chuàng)面上皮化情況亦為C組優(yōu)于B組優(yōu)于A組。結(jié)論自體ADSCs聯(lián)合無菌生物護(hù)創(chuàng)膜可明顯改善慢性創(chuàng)面修復(fù)細(xì)胞的增殖狀態(tài),促進(jìn)創(chuàng)面血管再生,改善創(chuàng)面局部生長環(huán)境,加速創(chuàng)面愈合。
[Abstract]:Objective to evaluate the effect of autogenous adipose-derived stem cells (ADSCs) combined with aseptic biofilm on chronic wound healing. Methods Sixty patients with chronic wound were randomly divided into two groups: group A (group A) and group B (group B), with 20 cases in each group. The healing time, healing time and wound healing rate of the 3 groups were observed. The proliferation of epithelial basal layer cells was observed at 7 days before treatment, 14 days after treatment and 50 days after treatment. The neovascularization density of epithelial basal layer was counted before and 7 days after treatment. Results compared with group A, the wound healing in group C was less inflammatory than that in group A, the granulation tissue formed well and the wound edge epithelium crawled faster, and the wound healing in group B was better than that in group A. The healing time of wound in group A, group B, and group C, the difference between the two groups was statistically significant. The rate of wound healing, the number of proliferative cells of epithelial basal layer and the density of neovascularization in group C were better than those in group B at different time points after treatment (P 0.05.3), and the density of neovascularization in group C was better than that in group A. The difference was statistically significant (P 0.05), and the epithelialization of wounds in group C was better than that in group B 50 days after treatment. Conclusion autogenous ADSCs combined with aseptic biological wound protective membrane can obviously improve the proliferation of chronic wound healing cells, promote wound vascular regeneration, improve the local growth environment of wound, and accelerate wound healing.
