建立纖維環(huán)穿刺法椎間盤退變模型

【摘要】:背景:目前雖然纖維環(huán)穿刺法構(gòu)建椎間盤退變模型的相關(guān)研究報(bào)道較多,但缺乏相對(duì)完整的影像、病理學(xué)以及細(xì)胞外基質(zhì)改變(蛋白多糖含量)的系統(tǒng)對(duì)比性研究。所以經(jīng)腹膜外入路暴露椎間隙纖維穿刺法造模的影像、病理學(xué)以及細(xì)胞外基質(zhì)改變是否與人類椎間盤退變類似需進(jìn)一步探討。目的:驗(yàn)證采用纖維環(huán)穿刺法建立椎間盤退變模型的可行性。方法:將30只新西蘭大白兔隨機(jī)分為2組,每組15只。纖維環(huán)穿刺組在經(jīng)腹膜外入路暴露L_(1,2),L_(2,3),_(3,4)椎間隙后,采用16號(hào)針頭穿刺;而假手術(shù)組只做切開不做穿刺。于造模術(shù)后4,8,16周2組均隨機(jī)抽取5只行MRI檢查后,以空氣栓塞法處死兔并切取椎間盤髓核組織,蘇木精-伊紅染色檢查組織學(xué)變化,Safranin O染色觀察髓核蛋白多糖水平改變。結(jié)果與結(jié)論:(1)MRI提示:纖維環(huán)穿刺組T2WI相椎間盤信號(hào)自造模術(shù)后開始降低,且隨著術(shù)后時(shí)間延長T2WI相椎間盤信號(hào)降低越明顯,于術(shù)后4周較假手術(shù)組有明顯改變,術(shù)后16周最明顯;假手術(shù)組術(shù)后各時(shí)間段改變不明顯;(2)蘇木精-伊紅染色顯示:纖維環(huán)穿刺組造模術(shù)后軟骨樣細(xì)胞減少,纖維樣細(xì)胞增多且所占比例增大,細(xì)胞外纖維排列紊亂,于第4周較假手術(shù)組有明顯改變,于第16周最明顯;假手術(shù)組術(shù)后各時(shí)間段改變不明顯;(3)Safranin O染色(髓核蛋白多糖水平):纖維環(huán)穿刺組組織著色于造模術(shù)后開始變淺,隨著術(shù)后時(shí)間延長著色變淺越明顯,于術(shù)后4周較假手術(shù)組有明顯改變,術(shù)后16周最明顯;(4)結(jié)果提示:纖維環(huán)穿刺法建立的椎間盤退變模型影像學(xué)表現(xiàn)及病理表現(xiàn)與人類椎間盤退變近似,提示纖維環(huán)穿刺法是建立椎間盤退變模型的可靠方法。
[Abstract]:Background: although there are many reports on the establishment of disc degeneration model by fiber loop puncture, there is a lack of systematic and comparative studies on the image, pathology and extracellular matrix changes (proteoglycan content). Therefore, the imaging, pathology and changes of extracellular matrix (ECM) of exposed intervertebral space fiber puncture via extraperitoneal approach need to be further explored. Objective: to verify the feasibility of establishing intervertebral disc degeneration model by fiber loop puncture. Methods: thirty New Zealand white rabbits were randomly divided into 2 groups with 15 rabbits in each group. In the fibrous loop puncture group, 16 needle puncture was used after the exposure of L1 / 2 / L / L / L / L / L / L / L / L / L / L / L / L through extraperitoneal approach, while in the sham operation group, only incision was made and no puncture was performed. After MRI examination, 5 rabbits in both groups were randomly selected at the 16th week after the operation. The rabbits were killed by air embolization and removed from the nucleus pulposus. The histopathological changes were examined by hematoxylin and eosin staining and the changes of nucleoproteoglycan levels were observed by Safranin O staining. Results and ConclusionMRI showed that the signal of intervertebral disc of T2WI phase in the fibrous annulus puncture group began to decrease after self-modeling, and the decrease of the signal of the intervertebral disc in T2WI phase was more obvious with the extension of postoperative time, and it was obviously changed at 4 weeks after operation compared with that in the sham operation group. The results of hematoxylin-eosin staining showed that chondroid cells decreased, fibroid cells increased and the proportion of fibroid cells increased in the sham operation group after 16 weeks. The arrangement of extracellular fibers was disordered, which was significantly changed in the 4th week than in the sham-operation group, and the most obvious in the 16th week. In the sham operation group, there was no obvious change of Safranin O staining at every time after operation (myelin proteoglycan level: the tissue of the fibrous ring puncture group began to become shallower after modeling, and the lighter the staining became with the extension of postoperative time. The results showed that the imaging and pathological manifestations of the degenerative model were similar to those of human intervertebral disc degeneration at 4 weeks after operation and 16 weeks after operation, and the pathological manifestations of the model were similar to those of human intervertebral disc degeneration. The results suggest that the method of fiber loop puncture is a reliable method to establish the model of disc degeneration.
