【摘要】：目的:觀察研究應(yīng)用選擇性直腸黏膜切除吻合術(shù)(TST)聯(lián)合消痔靈注射治療直腸黏膜脫垂(II、III度)的臨床療效。材料與方法:采用前瞻性設(shè)計(jì),對(duì)臨床上30例診斷為直腸黏膜脫垂(II、III度)的患者,均采用選擇性直腸黏膜切除吻合術(shù)(TST)聯(lián)合消痔靈注射的手術(shù)方法,通過吻合器選擇性切除松弛堆積的內(nèi)脫垂直腸黏膜,同時(shí)提拉懸吊黏膜,增加腸管張力;再在吻合口周圍點(diǎn)狀注射1:1消痔靈溶液,促使黏膜組織形成無菌性炎癥反應(yīng),進(jìn)一步鞏固了手術(shù)療效。術(shù)后常規(guī)對(duì)癥治療。通過自身對(duì)照方法及術(shù)后半年隨訪分析療效。數(shù)據(jù)應(yīng)用SPSS17.0統(tǒng)計(jì)軟件進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:應(yīng)用此方法治療30例II、III度直腸黏膜脫垂患者,治療后痊愈20例,顯效7例,有效3例,無效0例。術(shù)后半年后隨訪,復(fù)發(fā)0例。手術(shù)總有效率100.0%,療效滿意。結(jié)論:臨床應(yīng)用TST術(shù)聯(lián)合消痔靈注射方治療II、III度直腸黏膜脫垂,療效顯著,且具有手術(shù)操作簡單,損傷小,疼痛輕,術(shù)后恢復(fù)快的優(yōu)點(diǎn),值得臨床廣泛推廣應(yīng)用。
[Abstract]:Objective: to observe the clinical effect of selective rectal mucosal resection and anastomosis (TST) combined with Xiaozhiling injection in the treatment of rectal mucosal prolapse. Materials and methods: 30 patients with rectal mucosal prolapse were treated with selective rectal mucosal resection and anastomosis combined with Xiaozhiling injection in a prospective design. Selective resection of the loosely accumulated vertical intestinal mucosa through stapler, at the same time pulling suspended mucous membrane, increasing the tension of the intestinal canal, and then injecting 1:1 Xiaozhiling solution around the anastomotic stoma in the form of aseptic inflammatory reaction in the mucosal tissue. It further consolidated the curative effect of the operation. Postoperative routine symptomatic treatment. The therapeutic effect was analyzed by self-control method and half-year follow-up. The data were analyzed by SPSS17.0 software. Results: 30 cases of III degree rectal prolapse were treated with this method. 20 cases were cured, 7 cases were effective, 3 cases were effective, and 0 cases were ineffective. Follow up half a year after operation, recurrence in 0 cases. The total effective rate of operation was 100.00.The curative effect was satisfactory. Conclusion: the clinical application of TST combined with Xiaozhiling injection prescription in the treatment of rectal mucosal prolapse of III degree II is very effective and has the advantages of simple operation, small injury, light pain and quick postoperative recovery. It is worthy of extensive clinical application.
