【摘要】：目的通過比較成人心臟手術(shù)中HTK液和高鉀停搏液的心肌保護(hù)效果,探討最佳的心肌保護(hù)方法。方法檢索主要中外生物醫(yī)學(xué)數(shù)據(jù)庫(2005.10~2016.11)文獻(xiàn),收集HTK液與高鉀停搏液心肌保護(hù)比較的隨機(jī)對照研究,以Cochrane系統(tǒng)工具評價(jià)納入研究的偏倚風(fēng)險(xiǎn),對提取的結(jié)局指標(biāo)進(jìn)行Meta分析。結(jié)果最終納入研究13篇,共含1 047個(gè)病例,其中HTK組(實(shí)驗(yàn)組)502例,高鉀組(對照組)545例;各組研究經(jīng)檢驗(yàn)無統(tǒng)計(jì)學(xué)異質(zhì)性,采用固定模型合并。4項(xiàng)研究比較了兩組CK-MB峰值,兩組的CK-MB峰值無統(tǒng)計(jì)學(xué)差異(SMD=0.20,95%CI:-0.06~0.46,P=0.21);4項(xiàng)研究比較了兩組30d死亡率,兩組的30d死亡率無統(tǒng)計(jì)學(xué)差異(OR=0.53,95%CI:0.22~1.26,P=0.66);6項(xiàng)研究比較了兩組自動(dòng)復(fù)跳率,兩組的自動(dòng)復(fù)跳率無統(tǒng)計(jì)學(xué)差異(OR=1.42,95%CI:0.98~2.07,P=0.12)。結(jié)論成人心臟手術(shù)中,HTK液與高鉀停搏液心肌保護(hù)效果無差異。
[Abstract]:Objective to compare the myocardial protective effects of HTK solution and hyperkalemia cardioplegia solution in adult cardiac surgery. Methods A randomized controlled study on myocardial protection of HTK solution and hyperkalemic cardioplegia was collected by searching the main Chinese and foreign biomedical databases. The bias risk of the study was evaluated with Cochrane system tool, and the extracted outcome indexes were analyzed by Meta. Results there were 13 studies including 1 047 cases, including HTK group (502 cases) and hyperkalemia group (545 cases). There was no statistical heterogeneity in each group, and the peak value of CK-MB was compared between the two groups by using fixed model combined with 4 items of study. There was no statistical difference in the peak value of CK-MB between the two groups. There was no statistical difference between the two groups in the 30-day mortality rate of the two groups. There was no statistical difference in the 30-day mortality rate between the two groups. The two groups had no statistical difference in the 30-day mortality rate. The rate of automatic rebound was not significantly different between the two groups. There was no statistical difference between the two groups in the rate of automatic rebound. There was no statistical difference between the two groups in the rate of automatic rebound. Conclusion there is no difference in myocardial protection between HTK solution and hyperkalemia cardioplegia in adult cardiac surgery.
