【摘要】：目的:探討高頻超聲對(duì)肩袖全層撕裂亞型診斷的術(shù)前應(yīng)用價(jià)值。方法 :用過(guò)高頻超聲將岡上肌腱全層撕裂分為4型,并與肩關(guān)節(jié)鏡進(jìn)行對(duì)比觀察,評(píng)價(jià)超聲診斷岡上肌腱撕裂分型的符合率,并探討其在臨床中的應(yīng)用價(jià)值。結(jié)果:本次研究中65例患者的71個(gè)肩關(guān)節(jié)中,超聲發(fā)現(xiàn)岡上肌腱全層撕裂65個(gè),經(jīng)手術(shù)證實(shí)岡上肌腱全層撕裂67個(gè),超聲低估撕裂程度3個(gè),高估1個(gè)。在67個(gè)全層型撕裂中L型27個(gè),全層U型17個(gè)(其中雙U+L型1個(gè)),巨大撕裂15個(gè),新月型8個(gè)。超聲診斷岡上肌腱全層撕裂的敏感性為95.5%,特異性為75%,總符合率為94.4%。結(jié)論:高頻超聲可以很好的對(duì)肩袖全層撕裂進(jìn)行術(shù)前分型,對(duì)手術(shù)方式的選擇具有很高的指導(dǎo)價(jià)值。
[Abstract]:Objective: to evaluate the preoperative value of high-frequency ultrasound in the diagnosis of rotator cuff tear subtype. Methods: the full layer tear of supraspinatus tendon was divided into 4 types by ultra high frequency ultrasound and compared with shoulder arthroscopy to evaluate the coincidence rate of ultrasonic diagnosis of supraspinatus tendon tear and to explore its clinical application value. Results: 65 of the 71 shoulder joints of 65 patients were found to have supraspinar tendon tears by ultrasonography. 67 supracondylar tendon tears were proved by operation. Ultrasound underestimated the tear degree by 3 and overestimated 1. Among the 67 full-layer tears, 27 were L-type and 17 full-lamellar U-type (including 1 type of double U-L, 15 of giant tear and 8 of crescent type). The sensitivity and specificity of ultrasound in diagnosis of full-layer tear of supraspinatus tendon were 95.5and 750.The total coincidence rate was 94.4%. Conclusion: high frequency ultrasound can be used to classify the rotator cuff tears before operation, and it has a high guiding value for the choice of surgical methods.
