【摘要】：目的：使用系統(tǒng)評價的方法對經(jīng)皮球囊灌注骨水泥椎體成形單側(cè)入路與雙側(cè)入路方法進行分析,以期為椎體后凸成形入路及灌注骨水泥過程中的技術(shù)選擇提供依據(jù)。方法：計算機檢索Cochrane Library、PubMed、OVID、Embase、中國生物醫(yī)學(xué)文獻數(shù)據(jù)庫、萬方數(shù)據(jù)庫、維普資訊等中英文數(shù)據(jù)庫,手工檢索9種國內(nèi)相關(guān)雜志,檢索日期均從創(chuàng)刊至2014年3月,收集單側(cè)與雙側(cè)入路經(jīng)皮球囊灌注骨水泥椎體后凸成形修復(fù)骨質(zhì)疏松性椎體壓縮骨折的隨機對照試驗,由兩名系統(tǒng)評價員按納入與排除標準選擇試驗、提取資料和質(zhì)量評價后,并用RevMan 5.2軟件進行Meta分析。結(jié)果與結(jié)論：最終納入8個隨機對照試驗,共507例患者656個椎體。結(jié)果顯示,骨水泥滲漏方面兩組差異無顯著性意義；術(shù)前目測類比評分[WMD=-0.05(P=0.58)]、短期隨訪(≤3個月)[WMD=-0.02 (P=0.25)]與長期隨訪(≥12個月)[WMD=-0.08 (P=0.64)],兩組差異無顯著性意義；Oswestry功能障礙指數(shù)(ODI)短期隨訪[WMD=-0.13 (P=0.73)]和長期隨訪[WMD=-0.26 (P=0.81)],兩組差異無顯著性意義；骨水泥用量和操作時間兩組差異有顯著性意義,單側(cè)入路較雙側(cè)入路有優(yōu)勢；影像學(xué)結(jié)果中術(shù)前、術(shù)后后凸角及角度恢復(fù)值,差異均無顯著性意義提示單側(cè)和雙側(cè)入路經(jīng)皮球囊灌注骨水泥椎體后凸成形修復(fù)骨質(zhì)疏松性椎體壓縮骨折,在目測類比評分、影像學(xué)結(jié)果、并發(fā)癥方面無明顯差異；但在手術(shù)時間和放射暴露時間上,單側(cè)入路優(yōu)于雙側(cè)入路。
[Abstract]:Objective: to analyze the unilateral approach and bilateral approach of percutaneous balloon cementation in order to provide a basis for the selection of techniques in the process of kyphoplasty and cementation. Methods: the Chinese and English databases of Cochrane Library PubMeden OVIDB, Chinese Biomedical Literature Database, Wanfang Database and Weipu Information were searched by computer, and 9 domestic related magazines were searched manually from the beginning of publication to March 2014. A randomized controlled trial of unilateral and bilateral approach for the repair of osteoporotic vertebral compression fractures was carried out. The data and quality were extracted and evaluated by two systematic evaluators according to the criteria of inclusion and exclusion. Meta analysis was carried out with RevMan 5.2 software. Results & conclusion: a total of 507 patients with 656 vertebrae were enrolled in 8 randomized controlled trials.
