【摘要】：目的考察Bankart合并關(guān)節(jié)盂上唇從前至后部(superior labrum anterior to posterior,SLAP)損傷的復(fù)發(fā)性肩關(guān)節(jié)脫位患者采用關(guān)節(jié)鏡修復(fù)手術(shù)治療的臨床效果。方法回顧分析2011年5月至2015年1月間因復(fù)發(fā)性肩關(guān)節(jié)脫位接受關(guān)節(jié)鏡下Bankart聯(lián)合SLAP損傷修復(fù)治療的15例患者臨床資料,將其作為試驗(yàn)組研究對(duì)象�；颊咂骄挲g為24.2歲(16~38歲)。術(shù)中首先采用可吸收縫合錨釘對(duì)不穩(wěn)定的SLAP損傷進(jìn)行修復(fù)固定,之后再修復(fù)Bankart損傷。另選取15例單純Bankart損傷接受關(guān)節(jié)鏡修復(fù)治療的復(fù)發(fā)性肩關(guān)節(jié)脫位患者作為對(duì)照組研究對(duì)象,患者的平均年齡為24.6(18~35歲)。在術(shù)前及末次隨訪時(shí),采用視覺模擬評(píng)分(Visual Analogue Scale,VAS)、美國(guó)肩肘關(guān)節(jié)外科醫(yī)師評(píng)分(American Shoulder and Elbow Surgeons,ASES)、Rowe肩關(guān)節(jié)評(píng)分以及肩關(guān)節(jié)活動(dòng)度等對(duì)兩組患者臨床效果進(jìn)行評(píng)估。結(jié)果試驗(yàn)組的平均隨訪時(shí)間為15個(gè)月(13~28個(gè)月),對(duì)照組為22個(gè)月(21~34個(gè)月)。試驗(yàn)組平均VAS評(píng)分從術(shù)前4.9降低到末次隨訪時(shí)的1.9,差異有統(tǒng)計(jì)學(xué)意義(P0.05);平均ASES評(píng)分和Rowe肩關(guān)節(jié)評(píng)分分別從術(shù)前的33.7和56.4提高到末次隨訪時(shí)91.8和94.1,差異均有統(tǒng)計(jì)學(xué)意義(P0.05);在VAS評(píng)分、ASES評(píng)分以及Rowe評(píng)分方面,術(shù)前及術(shù)后隨訪時(shí),兩組之間差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。到末次隨訪時(shí),兩組所有病例均未再次出現(xiàn)復(fù)發(fā)性脫位,經(jīng)物理檢查亦未證實(shí)肩關(guān)節(jié)不穩(wěn)存在。兩組所有病例患側(cè)肩關(guān)節(jié)的前屈、外展、內(nèi)旋活動(dòng)度均恢復(fù)至正常水平。結(jié)論關(guān)節(jié)鏡下Bankart聯(lián)合SLAP損傷可吸收縫合錨釘固定修復(fù)治療復(fù)發(fā)性肩關(guān)節(jié)不穩(wěn)臨床效果滿意,可有效重建患者肩關(guān)節(jié)穩(wěn)定,改善肩關(guān)節(jié)功能。
[Abstract]:Objective to investigate the clinical effect of arthroscopic repair of recurrent dislocation of shoulder in patients with Bankart complicated with anterior and posterior superior labial superior labrum anterior to posteriorus injury. Methods from May 2011 to January 2015, the clinical data of 15 patients with recurrent shoulder dislocation treated by arthroscopic Bankart combined with SLAP injury repair were retrospectively analyzed. The average age of the patients was 24.2 years old and 16 to 38 years old. Absorbable suture anchors were first used to repair and fix unstable SLAP damage, and then Bankart damage was repaired. In addition, 15 patients with recurrent shoulder dislocation treated with arthroscopic repair of Bankart injury were selected as control group. The average age of the patients was 24. 6 and 1 835 years old. Before operation and at the last follow-up, the clinical effects of the two groups were evaluated by visual Analogue scale, American Shoulder and Elbow SurgeonsASESE Rowe shoulder joint score and shoulder motion. Results the mean follow-up time was 15 months (13 ~ 28 months) in the trial group and 21 ~ 34 months in the control group. The average VAS score of the trial group decreased from 4.9 before operation to 1.9 at the last follow-up, the difference was statistically significant (P 0.05), the average ASES score and the Rowe shoulder score increased from 33.7 and 56.4 before operation to 91.8 and 94.1 at the last follow-up, respectively. The difference was statistically significant. In terms of VAS score, ASES score and Rowe score, There was no significant difference between the two groups in preoperative and postoperative follow-up (P 0.05). At the last follow-up, no recurrent dislocation was found in all cases in both groups, and the shoulder joint instability was not confirmed by physical examination. The flexion, abduction and internal rotation of the shoulder joint of the two groups all returned to normal level. Conclusion arthroscopic Bankart combined with SLAP injury can be used to repair recurrent shoulder instability with absorbable suture anchor nail. It can effectively reconstruct the stability of shoulder joint and improve the function of shoulder joint.
