半導(dǎo)體激光局部照射對(duì)SD大鼠缺血皮瓣愈合時(shí)ICAM-1和MCP-1表達(dá)的影響

【摘要】:目的 觀察半導(dǎo)體激光局部照射SD大鼠缺血皮瓣創(chuàng)面愈合時(shí)組織中炎癥反應(yīng)因子細(xì)胞間黏附分子-1(intercellular cell adhesion molecule-1,ICAM-1)和單核細(xì)胞趨化蛋白-1(monocyte chemotactic protein-1,MCP-1)的表達(dá),探討其促進(jìn)缺血皮瓣愈合的機(jī)制。方法 在20只雄性SD大鼠背部?jī)蓚?cè)對(duì)稱各制備2個(gè)皮瓣,蒂位于尾側(cè),長(zhǎng)6 cm,寬1 cm,2個(gè)皮瓣間距離2 cm。實(shí)驗(yàn)側(cè)(實(shí)驗(yàn)組)皮瓣術(shù)后行半導(dǎo)體激光照射,每天1次,連續(xù)照射數(shù)天直至取材,同時(shí)遮擋對(duì)照側(cè)(對(duì)照組)。采用自身對(duì)照法比較實(shí)驗(yàn)組和對(duì)照組的創(chuàng)面愈合情況和皮瓣存活面積,分別于術(shù)后4、7、10、14 d取皮瓣中段組織進(jìn)行病理學(xué)染色,分析制備的大鼠皮瓣血管形成情況和炎癥反應(yīng)因子ICAM-1和MCP-1的表達(dá)。結(jié)果 皮瓣遠(yuǎn)端發(fā)生明顯的缺血壞死。術(shù)后7 d,實(shí)驗(yàn)組和對(duì)照組的皮瓣存活率分別為(84.6±10.8)%和(79.8±11.3)%,實(shí)驗(yàn)組皮瓣存活率明顯高于對(duì)照組(n=5,t=7.299,P=0.002)。術(shù)后4、7、10、14 d實(shí)驗(yàn)組微血管密度分別為(74.160±25.968)、(115.937±17.827)、(87.330±21.383)、(69.644±12.126)個(gè)/mm~2,均明顯高于對(duì)照組(33.460±8.310)個(gè)/mm~2(t=3.089,P=0.037)、(36.685±7.693)個(gè)/mm~2(t=11.741,P=0.000)、(43.474±7.400)個(gè)/mm~2(t=4.185,P=0.014)、37.104±8.721(t=5.218,P=0.006)。術(shù)后4 d實(shí)驗(yàn)組炎癥因子ICAM-1的表達(dá)量0.063±0.015明顯高于對(duì)照組0.045±0.017(t=8.430,P=0.001);術(shù)后7 d實(shí)驗(yàn)組炎癥因子ICAM-1和MCP-1的表達(dá)量分別為0.022±0.008、0.041±0.008,均明顯低于對(duì)照組0.042±0.015(t=4.309,P=0.013)、0.065±0.007(t=6.731,P=0.003)。結(jié)論 半導(dǎo)體激光局部照射SD大鼠缺血皮瓣能調(diào)節(jié)創(chuàng)面愈合時(shí)炎癥因子ICAM-1和MCP-1的表達(dá),是促進(jìn)缺血皮瓣愈合的重要機(jī)制之一。
[Abstract]:Objective to observe the expression of inflammatory response factor (cell adhesion) intercellular cell adhesion molecule-1 (ICAM-1) and monocyte chemoattractant protein-1 (MCP-1) in the wound healing of ischemic flap of SD rats after local irradiation with semiconductor laser, and to explore the mechanism of promoting the healing of ischemic flap. Methods two flaps were prepared in 20 male Sprague-Dawley rats. The pedicle was located at the caudal side, 6 cm in length and 1 cm in width, and the distance between the two flaps was 2 cm. The experimental side (experimental group) skin flap was irradiated by semiconductor laser once a day for several days, while the control side (control group) was occluded at the same time. The wound healing and flap survival area of the experimental group and the control group were compared with that of the control group by self-control method. The tissue of the middle segment of the flap was stained with histopathology on the 4th day and 1014 d after operation. The angiogenesis and the expression of inflammatory response factor (ICAM-1) and MCP-1 were analyzed. Results obvious ischemic necrosis occurred at the distal end of the flap. On the 7th day after operation, the survival rate of the flap in the experimental group and the control group was 84.6 鹵10.8% and 79.8 鹵11.3%, respectively.
