【摘要】：目的：反肩關(guān)節(jié)假體的出現(xiàn)為肱骨近端腫瘤切除后重建提供了一種新選擇,本文通過(guò)回顧性分析我院2年內(nèi)收治的使用反肩關(guān)節(jié)假體治療的肱骨近端腫瘤病例,分析其手術(shù)方式、并發(fā)癥、術(shù)后功能等。 方法：收集我院骨科腫瘤病區(qū)在2013年1月至2014年12月收治的臨床診斷為肱骨近端腫瘤,并通過(guò)反肩關(guān)節(jié)假體置換治療的病例資料共8例,男性3例,女性5例,年齡25～61歲,平均38歲,軟骨肉瘤2例,侵襲性骨巨細(xì)胞瘤6例。均行反肩關(guān)節(jié)(Zimmer Trabecular MetalTM)置換術(shù)治療。評(píng)價(jià)其并發(fā)癥、術(shù)后功能評(píng)分等,分析療效。 結(jié)果：患者隨訪3～26月,平均13月,所有患者均未出現(xiàn)腫瘤復(fù)發(fā),無(wú)關(guān)節(jié)脫位及假體松動(dòng)。術(shù)后功能根據(jù)肌肉骨骼腫瘤評(píng)分系統(tǒng)(MSTS=Musculoskeletal Tumor Society score)評(píng)分為88%～95%,平均92%,主動(dòng)外展為100°～175°,平均為155°,主動(dòng)前屈為115°～170°,平均為150°。 結(jié)論：反肩關(guān)節(jié)置換治療肱骨近端腫瘤術(shù)后近期功能良好,這種重建方式特別適合于肩袖切除的病例,其前提是三角肌功能的保留。反肩關(guān)節(jié)置換是肱骨近端腫瘤切除后重建的理想方式。
[Abstract]:Objective: the appearance of reverse shoulder prosthesis provides a new choice for reconstruction of proximal humerus tumor after resection. We retrospectively analyzed the cases of proximal humerus tumor treated with antihumeral prosthesis in our hospital within 2 years. The operative methods, complications and postoperative functions were analyzed. Methods: the clinical data of 8 patients with proximal humerus tumors admitted from January 2013 to December 2014 in our orthopedic tumor area were collected and treated by reverse shoulder prosthesis replacement. There were 3 males and 5 females aged 2561 years. The mean age was 38 years, 2 cases of chondrosarcoma and 6 cases of invasive giant cell tumor of bone. All patients were treated with Trabecular Trabecular replacement. To evaluate the complications, postoperative functional score, and analyze the curative effect. Results: all the patients were followed up for 3 ~ 26 months (mean 13 months). No recurrence of tumor, no dislocation of joint and loosening of prosthesis were found in all patients. According to the musculoSkeletal Tumor Society score) scoring system of musculoskeletal tumors, the postoperative function was 88 / 95 (mean 92 / 95), the active abduction was 100 擄/ 175 擄, the average was 155 擄, the active anterior flexion was 115 擄/ 170 擄and the average was 150 擄. Conclusion: the short term function of reverse shoulder replacement for proximal humerus tumor is good. This reconstruction method is especially suitable for patients with rotator cuff resection, and the premise is the preservation of deltoid muscle function. Reverse shoulder replacement is an ideal method for reconstruction after resection of proximal humerus tumor.
