新型停搏液對(duì)在體心臟冠脈血管內(nèi)皮保護(hù)效應(yīng)的實(shí)驗(yàn)研究

【摘要】:目的:本文通過(guò)研究一氧化氮(NO)、內(nèi)皮素(ET-1)和循環(huán)內(nèi)皮細(xì)胞(CEC)水平變化,以及冠脈舒縮功能變化,探討新型非去極化腺苷-利多卡因停搏液對(duì)在體犬冠脈血管內(nèi)皮細(xì)胞的保護(hù)作用及其可能機(jī)制。方法:本研究分為兩部分,第一部分建立犬體外循環(huán)模型并模擬臨床體外循環(huán)下心臟直視手術(shù),通過(guò)實(shí)驗(yàn)犬的生命體征和血流動(dòng)力學(xué)等指標(biāo)闡明犬體外循環(huán)手術(shù)的麻醉管理特點(diǎn);第二部分12只實(shí)驗(yàn)雜種犬隨機(jī)分為腺苷-利多卡因停搏液(Adenosine-lidocaine cardioplegia, AL組,n=6)和ST.thomasⅡ停搏液組(K組,n=6),測(cè)定術(shù)前(T1)、心臟復(fù)跳后1h(T2)和2h(T3)三個(gè)時(shí)點(diǎn)的一氧化氮(NO)、內(nèi)皮素(ET)和循環(huán)內(nèi)皮細(xì)胞(CEC)水平變化,并觀察冠脈血管舒張功能變化,及進(jìn)行透射電鏡下超微結(jié)構(gòu)分析。結(jié)果:兩組術(shù)前基礎(chǔ)值無(wú)統(tǒng)計(jì)學(xué)差異。T3時(shí)K組血漿中NO較AL組明顯減少(P0.05),ET明顯升高(P0.05),CEC明顯增多(P0.05)。K組冠脈血管舒張最大百分比29.13%±8.24%低于AL組68.52%士11.32%(P0.01)。電鏡結(jié)果提示:K組冠脈血管內(nèi)皮細(xì)胞超微結(jié)構(gòu)損傷較AL組更嚴(yán)重。結(jié)論:腺苷-利多卡因停搏液對(duì)犬冠脈血管內(nèi)皮細(xì)胞保護(hù)效果優(yōu)于ST.thomasⅡ停搏液。
[Abstract]:Objective: to study the changes of nitric oxide (no), endothelin (et 1) and circulating endothelial cell (CEC1) levels, and the changes of coronary systolic and diastolic function. To investigate the protective effect of a novel non-depolarized adenosine-lidocaine cardioplegic solution on canine coronary vascular endothelial cells (CAVEC) in vivo and its possible mechanism. Methods: this study was divided into two parts. The first part was to establish a canine cardiopulmonary bypass model and simulate the open heart surgery under clinical cardiopulmonary bypass.
