膝內(nèi)翻者步態(tài)的生物力學(xué)特征

【摘要】:研究目的:膝內(nèi)翻所帶來的危害越來越受人們關(guān)注,但是對(duì)膝內(nèi)翻者步態(tài)的研究卻較少見。本文從膝內(nèi)翻者步態(tài)的運(yùn)動(dòng)學(xué)特征以及動(dòng)力學(xué)特征,系統(tǒng)探討膝內(nèi)翻者步態(tài)的生物力學(xué)特征,為膝內(nèi)翻者的臨床診斷提供量化數(shù)據(jù),探究引起膝內(nèi)翻者損傷的可能機(jī)制,并且為康復(fù)提供理論依據(jù)。研究方法:本研究選取有膝內(nèi)翻的受試者13名,根據(jù)膝內(nèi)翻受試者的年齡、身高、體重、訓(xùn)練項(xiàng)目、下肢形態(tài)學(xué)指標(biāo)等特征匹配13名無膝內(nèi)翻的健康受試者。運(yùn)用Vicon-MX紅外動(dòng)作捕捉系統(tǒng)、kistler測力臺(tái)、Zebris步態(tài)分析跑臺(tái)FDM-TDS系統(tǒng)以及win-pod平衡及步態(tài)分析儀對(duì)膝內(nèi)翻組和非膝內(nèi)翻組進(jìn)行步態(tài)參數(shù)的采集,并分析時(shí)相、跨步長、足偏角、關(guān)節(jié)角度、足-地接觸力、足底壓力等生物力學(xué)指標(biāo)。對(duì)膝內(nèi)翻組和非膝內(nèi)翻組同側(cè)下肢步態(tài)的生物力學(xué)參數(shù)的對(duì)比,采用獨(dú)立樣本t檢驗(yàn);對(duì)受試者本身的左、右下肢對(duì)比,采用配對(duì)t檢驗(yàn)。研究結(jié)果:①受試者的左下肢和右下肢的各個(gè)步態(tài)指標(biāo)的差異均無統(tǒng)計(jì)學(xué)意義;②膝內(nèi)翻組與非膝內(nèi)翻組的各個(gè)時(shí)相參數(shù)的差異無統(tǒng)計(jì)學(xué)意義;③膝內(nèi)翻組的足偏角顯著低于非膝內(nèi)翻組(P0.05);兩組之間的ki寬、步長和跨步長等指標(biāo)的差異均無統(tǒng)計(jì)學(xué)意義(P0.05);④在支撐期,兩組之間的膝內(nèi)翻最大角度的差異具有統(tǒng)計(jì)學(xué)意義(P0.05);膝內(nèi)翻角度最大時(shí)所對(duì)應(yīng)的踝關(guān)節(jié)內(nèi)外翻角度的差異具有統(tǒng)計(jì)學(xué)意義(P0.05);其他指標(biāo)均無統(tǒng)計(jì)學(xué)意義;⑤膝內(nèi)翻組的足底內(nèi)側(cè)比值均大于非膝內(nèi)翻組,且兩組指標(biāo)間的差異有統(tǒng)計(jì)學(xué)意義(P0.05);足底外側(cè)比值均小于非膝內(nèi)翻組,且兩組指標(biāo)間的差異具有統(tǒng)計(jì)學(xué)意義(P0.05);動(dòng)態(tài)測試下的左右變量明顯大于非膝內(nèi)翻組,且差異具有統(tǒng)計(jì)學(xué)意義;⑥膝內(nèi)翻者在左右方向的力谷值/體重的數(shù)據(jù)具有顯著性差異(P0.05),其他足地接觸力參數(shù)兩組間差異無統(tǒng)計(jì)學(xué)意義(P0.05)。研究結(jié)論:①膝內(nèi)翻者在支撐期的最大內(nèi)翻角度顯著增大,并伴有步態(tài)的足偏角減小,表明膝內(nèi)翻改變了行走的正常步態(tài);②膝內(nèi)翻人群足底的內(nèi)側(cè)受力增大,外側(cè)受力減小,足-地接觸力在額狀面向內(nèi)的力值顯著增大,表明膝內(nèi)翻會(huì)改變步態(tài)的足地接觸力;③膝內(nèi)翻人群在膝關(guān)節(jié)最大內(nèi)翻角度時(shí),所對(duì)應(yīng)的踝關(guān)節(jié)出現(xiàn)外翻,表明行走時(shí)足出現(xiàn)外翻作為代償;④膝內(nèi)翻人群行走時(shí)步態(tài)線的左右變量變化較大,會(huì)影響身體平衡。
[Abstract]:Objective: the harm caused by genu varus has been paid more and more attention, but the gait of genu varus is rare. Based on the kinematic and dynamic characteristics of the genu varus gait, the biomechanical characteristics of the genu varus gait are systematically discussed in this paper, which can provide quantitative data for the clinical diagnosis of the genu varus and explore the possible mechanism of the genu varus injury. And to provide the theoretical basis for rehabilitation. Methods: thirteen subjects with genu varus were selected and 13 healthy subjects without varus were matched according to their age, height, weight, training items and morphological index of lower extremity. Gait analysis using Vicon-MX infrared motion capture system FDM-TDS system and win-pod balance and gait analyzer were used to collect gait parameters of genu varus group and non-genu varus group. The time phase, step length, foot angle and joint angle were analyzed. Foot-ground contact force, foot pressure and other biomechanical indicators. The biomechanical parameters of the ipsilateral lower limb gait in the genu varus group and the non-genu varus group were compared by independent sample t test, and the left and right lower extremities of the subjects were compared by paired t test. Results there was no significant difference in gait indexes between the left lower limb and the right lower extremity between the two groups. There was no significant difference in the time phase parameters between the genu varus group and the non-genu varus group. (3) the foot deflection angle in the genu varus group was significantly lower than that in the non-genu varus group (P 0.05), and there was no significant difference in the Ki width, step length and stride length between the two groups. The difference of the maximum angle of knee varus between the two groups was statistically significant (P 0.05), and that of the knee varus angle corresponding to the maximal angle of knee varus had statistical significance (P 0.05). (5) the medial plantar ratio in the genu varus group was higher than that in the non-genu varus group, and the difference between the two groups was statistically significant (P 0.05), and the lateral plantar ratio was lower than that in the non-genu varus group. The difference between the two groups was statistically significant (P 0.05), and the left and right variables in the dynamic test were significantly larger than those in the non-genu varus group, and the difference was statistically significant. (6) there was significant difference in the data of the force valley value / body weight of the knee varus in the left and right directions (P 0.05), but there was no significant difference between the other two groups in the contact force between the two groups (P 0.05). Conclusion: the maximal varus angle of the genu varus group increased significantly during the support period, and the foot deflection angle of the gait decreased, indicating that the medial force on the plantar increased and the lateral force decreased in the genu varus group, which showed that the knee varus changed the walking normal gait. The value of the foot-ground contact force in the frontal face increased significantly, which indicated that the knee varus would change the gait contact force. 3. When the knee joint maximal varus angle of the knee joint, the corresponding ankle joint appeared valgus. The results showed that the valgus of the foot was the variable of the walking gait line of the compensatory 4 knee varus group, which would affect the balance of the body.
