椎體成形術(shù)與椎體后凸成形術(shù)中骨水泥灌注壓力的比較研究

【摘要】:目的:比較椎體成形術(shù)與椎體后凸成形術(shù)中骨水泥灌注壓力的動(dòng)態(tài)變化。方法:1.設(shè)計(jì)與調(diào)試能直接記錄椎體成形術(shù)與椎體后凸成形術(shù)中骨水泥灌注壓力動(dòng)態(tài)變化的測(cè)量裝置。2.在微機(jī)控制電子萬(wàn)能材料試驗(yàn)機(jī)上以0.25mm/min、40N,0.25mm/min、60N,0.5mm/min、60N各2組共6組進(jìn)行驗(yàn)證試驗(yàn),得到電壓差與壓力之間的關(guān)系。3.將10個(gè)椎體標(biāo)本分成2組:PVP組(A1,A2,A3,A4,A5)、PKP組(B1,B2,B3,B4,B5),在室溫20℃下經(jīng)一側(cè)椎弓根PVP組行椎體成形術(shù)而PKP組行椎體后凸成形術(shù),骨水泥灌注量為2.4m L,灌注時(shí)間為48s,勻速地進(jìn)行骨水泥灌注,采用設(shè)計(jì)的灌注壓力測(cè)量裝置記錄骨水泥灌注時(shí)的實(shí)時(shí)動(dòng)態(tài)壓力。術(shù)后椎體行CT檢查,觀察骨水泥在椎體內(nèi)的彌散分布情況。結(jié)果:1.驗(yàn)證試驗(yàn)中電壓差與壓力呈線(xiàn)性關(guān)系,壓力隨電壓差的增加而變大,F(N)=440×U(V)。2.椎體后凸成形術(shù)中丙骨水泥的灌注壓力較椎體成形術(shù)中降低。在室溫20℃和推注速度為0.05m L/s下,PVP組第一管骨水泥的平均灌注壓力為12.41±2.42N,最大灌注壓力為19.89±3.82N,第二管骨水泥的平均灌注壓力為19.81±3.03N,最大灌注壓力為33.44±4.86N;而PKP組第一管骨水泥的平均灌注壓力為6.33±2.05N,最大灌注壓力為10.74±3.32N,第二管骨水泥的平均灌注壓力為9.42±1.69N,最大灌注壓力為18.18±1.77N。結(jié)論:1.椎體后凸成形術(shù)中骨水泥在椎體內(nèi)灌注壓力較椎體成形術(shù)中降低。2.椎體后凸成形術(shù)中骨水泥首先填充球囊擴(kuò)張留下的空腔,灌注壓力較低,彌散情況較椎體成形術(shù)中更為均勻,利于降低術(shù)中骨水泥滲漏的發(fā)生率。
[Abstract]:Objective: to compare the dynamic changes of bone cement perfusion pressure between vertebroplasty and kyphoplasty. Method 1: 1. Design and debug a measuring device that can directly record the dynamic changes of bone cement perfusion pressure in vertebroplasty and kyphoplasty. In the microcomputer controlled electronic universal material testing machine, the verification tests were carried out in 6 groups with 0.25mm / min 40N / min 40mm / min 60 mm / min 60N and 0.5mm / min 60N, respectively.
