【摘要】：目的觀察半肩關(guān)節(jié)置換術(shù)治療骨質(zhì)疏松性肱骨近端粉碎性骨折的臨床效果。方法 26例高齡(年齡60歲)骨質(zhì)疏松性肱骨近端粉碎性骨折患者,均采用半肩關(guān)節(jié)置換術(shù)聯(lián)合抗骨質(zhì)疏松藥物治療。出院后每隔3個月隨訪1次。觀察患者術(shù)后大小結(jié)節(jié)骨性愈合情況、肩關(guān)節(jié)假體位置。術(shù)后6個月采用雙能X線測量患者左股骨頸骨密度。末次隨訪時采用肱骨近端骨折Neer評分及Constant評分系統(tǒng)評價患者肩關(guān)節(jié)功能。結(jié)果本組均得到隨訪,隨訪時間6~60個月,平均34.2個月。影像學(xué)檢查所有患者大小結(jié)節(jié)均骨性愈合,假體無松動、下沉、脫位。術(shù)前及術(shù)后6個月患者左股骨頸骨密度分別為(0.67±0.05)、(0.87±0.07)g/cm2,二者相比,P0.05。末次隨訪時26例患者Neer評分為優(yōu)15例、良8例、可3例、優(yōu)良率88.5%;術(shù)前及末次隨訪時Constant評分分別為(63.2±5.4)、(66.8±6.5)分,二者相比,P0.05。結(jié)論半肩關(guān)節(jié)置換術(shù)治療骨質(zhì)疏松性肱骨近端粉碎性骨折效果較好。
[Abstract]:Objective to observe the clinical effect of hemishoulder arthroplasty in the treatment of osteoporotic proximal humeral comminuted fracture. Methods 26 cases of osteoporotic proximal humeral comminuted fracture were treated with hemishoulder arthroplasty combined with anti-osteoporosis drugs. The patients were followed up every 3 months after discharge. The bone healing and the position of shoulder prosthesis were observed. Bone mineral density (BMD) of left femoral neck was measured 6 months after operation by dual energy X ray. At the last follow-up, Neer and Constant scoring system were used to evaluate the shoulder function of patients with proximal humerus fracture. Results all patients were followed up for 6 ~ 60 months (mean 34.2 months). All patients had bony healing and no loosening, sinking and dislocation of the prosthesis. The bone mineral density of left femoral neck was 0.67 鹵0.05 鹵0.87 鹵0.07 g / cm ~ 2 before operation and 6 months after operation, respectively. At the last follow-up, the Neer scores of 26 patients were excellent in 15 cases, good in 8 cases, fair in 3 cases, and excellent and good rate 88.5.The Constant scores before and at the last follow-up were 63.2 鹵5.4 鹵6.5, respectively (P 0.05). Conclusion half shoulder arthroplasty is effective in treating osteoporotic proximal humerus comminuted fracture.
