【摘要】：目的探討自體脂肪顆粒移植術(shù)在改善面部凹陷和面部皺紋等面部整形中的各個步驟的操作方法及其要點(diǎn),使自體脂肪顆粒移植術(shù)在面部整形中盡量獲得最佳的效果。方法收集2014年10月至2017年1月間在某整形機(jī)構(gòu)行自體脂肪顆粒移植術(shù)改善面部凹陷和面部皺紋的女性患者32例,年齡在25～57歲(平均年齡41歲)。根據(jù)患者需求,通過注射器行負(fù)壓抽取術(shù)選擇不同部位的成熟的皮下脂肪,將負(fù)壓吸出的脂肪經(jīng)過漂洗、沉淀等純化處理后,多層次、多隧道地注射至面部需要填充的部位。結(jié)果32例患者共注射62個部位,顳部13例,額部9例,顴頰部11例,鼻唇溝14例,下頦部5例,改善面部皺紋10例,注射脂肪填充劑量2～30ml/次,每個部位注射1～3次,兩次注射間隔3～6個月。每個部位均過量矯正約40%。根據(jù)患者不同述求,供區(qū)分別為:腰部6例(18.75%),腹部14例(43.75%),臀部7例(21.875%),腿部5例(15.625%)。隨訪3～12個月后,患者顏面部的外觀有明顯的改善,面部輪廓豐滿充盈,自然流暢。本組32例患者均未發(fā)生感染、血腫、皮膚破潰、脂肪栓塞等并發(fā)癥。合計(jì)共注射62個部位,其中有58個部位術(shù)后效果表示滿意,4個部位的患者表示一般滿意,滿意率為93.55%。無患者對術(shù)后效果表示不滿意。其中注射至額部的滿意率為88.89%;顳部為92.31%;顴頰部是90.91%;鼻唇溝為92.86%;下頦部和面部皺紋的滿意率均為 100%。結(jié)論①自體脂肪顆粒移植術(shù)在作為目前常用來填充面目凹陷或改善皺紋的方法,具有操作簡便、取材來源豐富,患者滿意率高的優(yōu)點(diǎn)。②自體脂肪顆粒移植術(shù)是一種安全有效,微創(chuàng)無痕,并且可以達(dá)到既可以使面部輪廓更年輕化又可以減肥塑身的美容效果,值得在改善面部凹陷和面部皺紋等面部整形中推廣應(yīng)用。
[Abstract]:Objective to explore the operative methods and key points of autogenous fat granule transplantation in the improvement of facial depression and facial wrinkles, so as to obtain the best effect in facial plastic surgery. Methods from October 2014 to January 2017, 32 female patients with facial depression and facial wrinkles were treated with autologous fat particle transplantation in a plastic surgery institution. The age was 25 ~ 57 years (mean 41 years). According to the needs of patients, the mature subcutaneous fat of different parts was selected by negative pressure extraction with syringe. After rinsing, precipitating and other purification, the fat was injected into the area of facial filling by multi-level and multi-tunnel. Results Sixty-two sites were injected into 32 cases, temporal 13 cases, frontal 9 cases, zygomatic-buccal 11 cases, nasolabial sulcus 14 cases, chin 5 cases, facial wrinkles improved 10 cases, fat filling dose 2~30ml/ times, 3 times per site. The interval between two injections was 3 ~ 6 months. Each part was overcorrected by about 40%. According to the different description of the patients, the donor areas were as follows: the waist 6 cases with 18. 75A, the abdomen 14 cases with 43.75 stomach, the buttocks 7 cases with 21.875 5, the legs 5 cases with 15. 625 cases. After 3 ~ 12 months follow-up, the facial appearance was improved obviously, the facial contour was fullness and smooth. There were no complications such as infection, hematoma, skin rupture and fat embolism in 32 cases. A total of 62 sites were injected, of which 58 were satisfactory, and 4 were generally satisfactory, the satisfaction rate was 93.55. No patient was dissatisfied with the effect of the operation. The satisfaction rate of injection into frontal part was 88.89; that of temporal part was 92.31; that of zygomatic-buccal part was 90.91; that of nasolabial sulcus was 92.86; the satisfaction rate of chin and facial wrinkles was 100. Conclusion (1) autologous fat granule transplantation is a safe and effective method for filling face depression or improving wrinkle, which has the advantages of simple operation, abundant source of materials and high satisfaction rate of patients. It can not only make facial contour younger, but also lose weight and plastic body. It is worth popularizing and applying in improving facial depression and facial wrinkles.
