【摘要】：目的:通過(guò)本研究揭示CTRP12在CAD中的變化及與七氟醚的關(guān)系篩查CTRP12作為預(yù)示冠心病臨床檢測(cè)指標(biāo)的可能性,并明確CTRP12的心臟保護(hù)效應(yīng)。方法:隨機(jī)選取健康體檢志愿者62例為對(duì)照組(N組),選取心內(nèi)科經(jīng)冠脈造影證實(shí)CAD患者67例為冠心病組(C組),選取心外科預(yù)備行非體外循環(huán)的冠脈旁路移植術(shù)(OPCABG)患者52例分為丙泊酚組(P組)和丙泊酚+七氟醚組(S組),所有受試者均需禁食8小時(shí)以上(手術(shù)組還需同時(shí)禁飲6至少小時(shí))并于次日清晨采集靜脈血3~5ml,P組和S組須另外采集術(shù)后4h靜脈血各5ml,設(shè)定在1000轉(zhuǎn)/分的轉(zhuǎn)速離心15分鐘,用移液槍吸取上層血漿,分裝放入設(shè)定溫度在-70℃的冰箱凍存。記錄N組、C組的年齡、性別、吸煙率、飲酒率、糖尿病患病率、高血壓患病率、BMI、TC、TG、HDL-C、LDL-C、FBG以及CRP,并采用酶聯(lián)免疫吸附法檢測(cè)C組和N組的CTRP12、IL-1β;采用酶聯(lián)免疫吸附法檢測(cè)P組和S組術(shù)前P0、S0及術(shù)后P1、S1四個(gè)時(shí)間點(diǎn)的CTRP12濃度,并計(jì)算P0和P1,S0和S1的差值dP和dS。結(jié)果:所有數(shù)據(jù)用SPSS軟件分析。結(jié)果得出冠心病組CTRP12與降低的IL-1β、HDL-C和增高的BMI呈正相關(guān)。在N、C、P、S組受試對(duì)象中CTRP12血漿水平與高血壓患病率和BMI呈正相關(guān),與HDL-C和IL-1β呈負(fù)相關(guān)。logistic回歸分析CTRP12是冠心病的獨(dú)立危險(xiǎn)因素。與N組相比C組CTRP12濃度明顯升高,且兩者的差異有統(tǒng)計(jì)學(xué)意義。S組和P組術(shù)前CTRP12濃度無(wú)差異,P組CTRP12濃度在誘導(dǎo)前與術(shù)后4小時(shí)的差值dP低于S組誘導(dǎo)前與術(shù)后4小時(shí)的差值dS,且具有統(tǒng)計(jì)學(xué)差異。結(jié)論:目前的研究顯示,血清CTRP12濃度可以為預(yù)測(cè)冠心病患病率提供參考。七氟醚的心肌保護(hù)作用可能與CTRP12有關(guān)。
[Abstract]:Objective: to investigate the changes of CTRP12 in CAD and the relationship between CTRP12 and sevoflurane (sevoflurane) as a predictor of coronary heart disease (CHD), and to determine the cardioprotective effect of CTRP12. Methods: 62 healthy volunteers were randomly selected as control group, 67 patients with CAD confirmed by coronary angiography in cardiology were selected as group C, and cardiac surgery was performed with off-pump coronary artery bypass grafting (OPCABG). 52 patients were divided into propofol group (group P) and propofol sevoflurane group (group S). All subjects had to fast for more than 8 hours (the operation group had to stop drinking for at least 6 hours at the same time) and collected venous blood in the next morning. Group A should collect 5 ml venous blood 4 hours after operation and centrifuge for 15 minutes at a rotational speed of 1000 rpm / min.
