【摘要】：目的:對比腹腔鏡輔助與傳統(tǒng)開腹回腸袢式造口還納術(shù),評估腹腔鏡技術(shù)在回腸袢式造口還納術(shù)中應(yīng)用的優(yōu)劣。方法:對本院接受腹腔鏡輔助(n=25)和常規(guī)開腹手術(shù)(n=41)的回腸袢式造口患者的基線資料、術(shù)中出血量、手術(shù)時(shí)間、術(shù)后住院天數(shù)、術(shù)后并發(fā)癥等進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:2組患者的基線臨床資料無統(tǒng)計(jì)學(xué)差異,腹腔鏡輔助組術(shù)后總并發(fā)癥發(fā)生率4%(1/25),為頑固性腹水1例,無切口感染及腸梗阻(0/25);傳統(tǒng)手術(shù)組頑固性腹水、切口感染和腸梗阻的發(fā)生率分別為14.63%(6/41)、4.88%(2/41)、9.76%(4/41)。腹腔鏡輔助組手術(shù)時(shí)間顯著長于傳統(tǒng)手術(shù)組(P0.05)。術(shù)中出血量及術(shù)后住院天數(shù),2組間無顯著性差異。結(jié)論:腹腔鏡輔助回腸袢式造口還納術(shù)是安全的微創(chuàng)手術(shù)方式,有減少術(shù)后并發(fā)癥的趨勢。
[Abstract]:Objective: to evaluate the advantages and disadvantages of laparoscopy in ileal loop anastomosis. Methods: the baseline data, intraoperative bleeding volume, operative time, postoperative hospital stay and postoperative complications of the patients undergoing laparoscopically assisted operation and routine laparotomy were analyzed statistically. Results there was no significant difference in baseline clinical data between the two groups. The incidence of postoperative complications in the laparoscopy assisted group was 4 / 25%, one case of intractable ascites without incisional infection and intestinal obstruction. The incidence of incisional infection and intestinal obstruction were 14.63 / 41 / 4.88 / 2 / 41 / 9.76 / 4 / 41 respectively. The operative time of laparoscopy assisted group was significantly longer than that of traditional operation group (P 0.05). There was no significant difference in intraoperative bleeding and postoperative hospital stay between the two groups. Conclusion: laparoscopically assisted ileal loop ostomy is a safe and minimally invasive procedure with a tendency to reduce postoperative complications.
