【摘要】：目的:對(duì)內(nèi)側(cè)半月板后角損傷的患者,采用關(guān)節(jié)鏡下縫合修復(fù)方法,輔助內(nèi)側(cè)副韌帶淺層結(jié)構(gòu)局部松解,觀察其術(shù)后療效。方法:2014年3月至2014年10月,連續(xù)37例患者入選本研究。男23例,女14例,平均年齡36.7±9.2歲。單純內(nèi)側(cè)半月板損傷6例,前交叉韌帶合并內(nèi)側(cè)半月板損傷31例�？p合修復(fù)半月板時(shí),對(duì)內(nèi)側(cè)副韌帶淺層結(jié)構(gòu)的后方纖維進(jìn)行松解。半月板修復(fù)均采用自內(nèi)向外縫合方法。術(shù)后隨訪時(shí)評(píng)估患者主觀癥狀,有無關(guān)節(jié)交鎖、內(nèi)側(cè)關(guān)節(jié)間隙疼痛,有無壓痛,內(nèi)側(cè)副韌帶穩(wěn)定性。拍攝膝關(guān)節(jié)核磁共振片了解半月板愈合情況。結(jié)果:37例患者平均獲得26.1±4.4個(gè)月的隨訪。31例患者為前交叉韌帶損傷合并內(nèi)側(cè)半月板損傷,術(shù)后隨訪時(shí)膝關(guān)節(jié)穩(wěn)定性KT-1000檢查側(cè)-側(cè)差值平均為1.3±0.7 mm,與術(shù)前(7.1±2.2 mm)相比有顯著差異(P0.01)。另6例患者接受單純半月板縫合修復(fù),膝關(guān)節(jié)無不穩(wěn)定表現(xiàn)。全部患者膝關(guān)節(jié)內(nèi)側(cè)穩(wěn)定性術(shù)后較術(shù)前無顯著差異。3例患者主訴內(nèi)側(cè)關(guān)節(jié)間隙疼痛,無交鎖現(xiàn)象。術(shù)后核磁共振片顯示內(nèi)側(cè)半月板愈合率為81.1%。結(jié)論:關(guān)節(jié)鏡下縫合修復(fù)方法治療內(nèi)側(cè)半月板損傷,術(shù)后平均隨訪26.1個(gè)月,愈合率為81.1%。術(shù)中輔助內(nèi)側(cè)副韌帶淺層組織局部松解,可以改善視野、提高操作效率,術(shù)后患者無內(nèi)側(cè)結(jié)構(gòu)不穩(wěn)定表現(xiàn)。
[Abstract]:Objective: to observe the effect of arthroscopic suture repair on the superficial structure of medial collateral ligament in patients with posterior horn injury of medial meniscus. Methods: from March 2014 to October 2014, 37 consecutive patients were enrolled in this study. There were 23 males and 14 females with an average age of 36.7 鹵9.2 years. There were 6 cases of medial meniscus injury and 31 cases of anterior cruciate ligament combined with medial meniscus injury. The posterior fibers of the superficial structure of medial collateral ligament were loosened when meniscus was repaired by suture. Meniscus prostheses were sutured from inside to outside. The subjective symptoms, joint interlocking, pain in medial joint space, tenderness and stability of medial collateral ligament were evaluated during follow-up. To study meniscus healing by taking MRI film of knee joint. Results the mean follow-up time of 37 cases was 26.1 鹵4.4 months. 31 cases had anterior cruciate ligament injury combined with medial meniscus injury. The lateral side difference of knee joint stability in KT-1000 was 1.3 鹵0.7 mm after follow-up, which was significantly different from that before operation (7.1 鹵2.2 mm). The other 6 patients underwent meniscus suture and no instability of knee joint. There was no significant difference in medial knee joint stability after operation. 3 patients complained of medial joint space pain without locking. MRI showed that the healing rate of medial meniscus was 81.1%. Conclusion: arthroscopic suture repair for medial meniscus injury was followed up for an average of 26.1 months and the healing rate was 81.1%. The local release of medial collateral ligament can improve the visual field and improve the operation efficiency. There is no instability of medial structure in patients after operation.
