【摘要】：目的:探討腓骨截骨手術對膝骨性關節(jié)炎患者的近期臨床治療效果。方法:自2015年1月起,我科收治以內側間隙疼痛為主要表現(xiàn)的膝骨性關節(jié)炎患者12例(15膝);全部為女性患者,年齡48~72歲,平均年齡61.3歲;膝骨關節(jié)炎發(fā)病時間0.5~10年,平均5.3年。均采用腓骨截骨手術,即于患肢同側腓骨小頭下6-10cm處截取2-3cm腓骨對其膝關節(jié)骨性關節(jié)炎進行治療。術后1天下地適當進行關節(jié)活動,對患者手術前后進行相應影像學評估,并觀察記錄以下指標:術前、術后2天、術后2月VAS、HSS、KSS評分,分析各項指標變化情況。結果:1.所有患者住院天數(shù)平均6天;單側手術時間為25~55分鐘,平均單側手術時間33.3分鐘,術中均無明顯出血。術后均未出現(xiàn)傷口感染、延遲愈合及神經(jīng)損傷等并發(fā)癥。2.所有患者均于術后1天下地行走,主訴原有膝關節(jié)內側間隙疼痛明顯好轉或消失。術后2天復查膝關節(jié)負重位X線片,與術前膝關節(jié)負重位X線片比較,出現(xiàn)不同程度膝關節(jié)內側間隙增高。3.患者手術后2天與術前相比,術后患者的VAS評分、HSS評分及KSS臨床評分三項較術前明顯好轉(P0.05),患者手術后2月與術前相比,術后患者的VAS評分、HSS評分、KSS臨床評分及KSS功能評分四項較術前均明顯好轉(P0.05)。結論:腓骨截骨術手術操作簡單,創(chuàng)傷小,術后疼痛減輕明顯,關節(jié)功能恢復良好,是治療內翻型膝骨性關節(jié)炎的有效方法。
[Abstract]:Objective: to investigate the short-term clinical effect of fibula osteotomy on knee osteoarthritis. Methods: from January 2015, 12 cases of knee osteoarthritis with medial space pain as the main manifestation were treated in our department, all of them were female patients, aged 48 ~ 72 years, with an average age of 61.3 years, and the onset time of knee osteoarthritis was 0.5 ~ 10 years (mean 5.3 years). All patients were treated with fibula osteotomy, in which 2-3cm fibula was amputated at the 6-10cm of the ipsilateral fibula under the head of fibula to treat osteoarthritis of the knee. One day after operation, appropriate joint movement was carried out, and the corresponding imaging evaluation was performed before and after operation. The following indexes were observed and recorded: before, 2 days after operation, 2 days after operation, VAS-HSS-KSS score, and the changes of each index were analyzed. The result is 1: 1. The average hospitalization time of all patients was 6 days, the time of unilateral operation was 2555 minutes and the average time of unilateral operation was 33.3 minutes. No postoperative complications such as wound infection, delayed healing and nerve injury were found. All the patients walked on the ground 1 day after operation and complained that the pain in the medial space of the original knee was obviously improved or disappeared. The results showed that the medial space of knee joint increased at different degrees compared with the preoperative radiographs of knee joint loading position 2 days after operation. Two days after operation, VAS scores and KSS clinical scores were significantly improved in patients 2 days after operation compared with those before operation (P 0.05), and compared with those before operation at 2 months after operation. The VAS scores, clinical scores and KSS functional scores were significantly improved after operation than those before operation (P 0.05). Conclusion: fibula osteotomy is an effective method for the treatment of varus knee osteoarthritis.
