【摘要】：目的探討誘導(dǎo)膜在感染性骨缺損治療中的作用。方法 2008年1月~2015年9月,應(yīng)用萬(wàn)古霉素骨水泥填塞、二期植骨治療感染性骨缺損38例。分為誘導(dǎo)膜包裹組(A組,24例)和無(wú)誘導(dǎo)膜包裹組(B組,14例)。記錄骨愈合、感染復(fù)發(fā)和再次手術(shù)干預(yù)情況,骨愈合和鄰近關(guān)節(jié)功能分級(jí)按Paley方法分別評(píng)價(jià)。結(jié)果隨訪12~51(平均19.6)個(gè)月。A組均一期愈合,臨床愈合時(shí)間3.0~7.0(平均5.25)個(gè)月;B組1例骨不愈合,B組臨床愈合時(shí)間3.0~15.0(平均6.64)個(gè)月。A組術(shù)后感染復(fù)發(fā)1例,B組感染復(fù)發(fā)3例。B組需要再次手術(shù)干預(yù)3例,A組無(wú)。兩組的骨愈合時(shí)間和再次手術(shù)干預(yù)率比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05),雖然兩組感染復(fù)發(fā)率比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),但B組感染復(fù)發(fā)率呈高于A組的趨勢(shì)。末次隨訪,兩組的骨缺損愈合分級(jí)和鄰近關(guān)節(jié)功能恢復(fù)比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論誘導(dǎo)膜完整性與骨愈合效果有關(guān),負(fù)載抗生素骨水泥填塞形成的誘導(dǎo)膜對(duì)感染性骨缺損術(shù)后感染復(fù)發(fā)具有一定的隔離保護(hù)作用。
[Abstract]:Objective to investigate the role of induction membrane in the treatment of infectious bone defect.Methods from January 2008 to September 2015, 38 cases of infectious bone defects were treated with vancomycin bone cement tamponade and secondary bone graft.Group A (n = 24) and group B (n = 14) were divided into two groups: group A (n = 24) and group B (n = 14).Bone healing, infection recurrence and re-operative intervention were recorded. Bone healing and adjacent joint function grades were evaluated by Paley method.Results all the patients in group A were healed at the first stage after a follow-up of 12 months (mean 19.6 months).The clinical healing time of group B was 3. 0 to 7. 0 (mean 5. 25) months. The clinical healing time of group B was 3. 0 to 15. 0 (mean, 6. 64) months. Group A had 1 recurrence of infection after operation.The time of bone healing and the rate of re-operation intervention were significantly different between the two groups. Although there was no significant difference in the recurrence rate of infection between the two groups, the recurrence rate of infection in group B was higher than that in group A.At the last follow-up, there was no significant difference in the grade of bone defect healing and the functional recovery of adjacent joints between the two groups (P 0.05).Conclusion the integrity of the induced membrane is related to the effect of bone healing. The induced membrane formed by the loading of antibiotic bone cement has a certain protective effect on the recurrence of infection after infectious bone defect.
