【摘要】：目的探討空心拉力螺釘(Hollow Tension Screw)、鎖定鋼板(Locking Plate)治療后踝骨折的近遠(yuǎn)期功能恢復(fù)情況。方法本研究病例皆為自2013.1~2014.1期間山東中醫(yī)藥大學(xué)附屬醫(yī)院山東省中醫(yī)院創(chuàng)傷骨科病房收治的44例后踝骨折手術(shù)病人。按內(nèi)固定方式的不同分為兩組:空心拉力螺釘內(nèi)固定組21例,鎖定鋼板內(nèi)固定組23例,隨訪術(shù)后1個(gè)月~半年。通過(guò)對(duì)兩組不同手術(shù)方式患者的手術(shù)時(shí)間、術(shù)中出血量、骨折臨床愈合時(shí)間、術(shù)后功能恢復(fù)情況進(jìn)行對(duì)比分析,經(jīng)統(tǒng)計(jì)學(xué)處理得出最后結(jié)果。結(jié)果兩組不同手術(shù)方式手術(shù)時(shí)間無(wú)顯著性差異(p0.05),手術(shù)過(guò)程中平均出血量有顯著性差異(p0.05),骨折臨床愈合時(shí)間有顯著性差異(p0.05)。兩者近期功能恢復(fù)情況有顯著性差異(p0.05),鎖定鋼板組近期功能恢復(fù)情況較空心拉力螺釘好,兩者遠(yuǎn)期功能恢復(fù)情況無(wú)顯著性差異(p0.05)。說(shuō)明兩種手術(shù)方式相比,手術(shù)時(shí)間和遠(yuǎn)期功能恢復(fù)情況差別不大,空心拉力螺釘組較鎖定鋼板組出血量少、骨折臨床愈合時(shí)間快,鎖定鋼板組較空心拉力螺釘組術(shù)后近期功能恢復(fù)情況好。結(jié)論兩者近期功能恢復(fù)情況有顯著區(qū)別;其中,鎖定鋼板內(nèi)固定術(shù)后近期功能恢復(fù)情況較空心拉力螺釘內(nèi)固定好,兩種固定方式遠(yuǎn)期功能恢復(fù)情況無(wú)顯著差異,兩者均是治療后踝骨折常用手術(shù)方式并且術(shù)后效果也是較為令人滿意的內(nèi)固定方式,具有固定牢固、操作簡(jiǎn)易和成功率高等優(yōu)勢(shì)。權(quán)衡各方面利弊與綜合考慮后,筆者認(rèn)為鎖定鋼板內(nèi)固定較空心拉力螺釘內(nèi)固定好。
[Abstract]:Objective to investigate the short and long term functional recovery of ankle fracture treated with Hollow Tension screwscrew and locking plate.Methods 44 cases of posterior ankle fracture were treated in the Department of Trauma and Orthopaedics, Shandong Provincial Hospital of traditional Chinese Medicine, Shandong University of traditional Chinese Medicine, from March 1 to January 2014.1.They were divided into two groups according to the different internal fixation methods: the internal fixation group (21 cases) and the locking plate group (23 cases). The patients were followed up for 1 month to 6 months after operation.The operation time, intraoperative bleeding amount, fracture healing time and postoperative functional recovery were compared and analyzed between the two groups, and the final results were obtained by statistical analysis.Results there was no significant difference in the operative time between the two groups in different operation methods (p0.05). There was a significant difference in the average amount of blood loss during the operation and the time of fracture healing (p0.05).There was a significant difference in the short-term functional recovery between the two groups (p0.05). The short-term functional recovery of the locking plate group was better than that of the hollow lag screw. There was no significant difference in the long-term functional recovery between the two groups.The results showed that there was no significant difference in the time of operation and the recovery of long-term function between the two types of operation. The bleeding volume of the hollow screw group was less than that of the locking plate group, and the healing time of the fracture was faster than that of the locking plate group.The short-term functional recovery of locking plate group was better than that of hollow lag screw group.Conclusion there is a significant difference in short-term functional recovery between the two groups, among which, the short-term functional recovery after locking plate fixation is better than that with hollow lag screw fixation, but there is no significant difference between the two fixation methods in long-term functional recovery.Both of them are common surgical methods for the treatment of ankle fracture and the postoperative effect is also satisfactory. They have the advantages of firm fixation, simple operation and high success rate.After weighing the advantages and disadvantages, the author thinks that locking plate fixation is better than hollow screw fixation.
