【摘要】：目的比較后側(cè)支撐板固定與前向后空心釘固定治療脛骨后pilon骨折的臨床療效。方法選取2012年2月—2016年2月在本院治療的脛骨后pilon骨折患者共83例,根據(jù)固定方法分為支撐板組47例和空心釘組36例。比較分析兩組患者的手術(shù)時(shí)間、術(shù)后住院時(shí)間、完全負(fù)重時(shí)間及骨折愈合時(shí)間;采用美國足踝外科協(xié)會(huì)(AOFAS)評(píng)分、VAS評(píng)分及AO影像學(xué)評(píng)分評(píng)價(jià)踝關(guān)節(jié)術(shù)后1年的功能;比較兩組患者的術(shù)后相關(guān)并發(fā)癥。結(jié)果兩組患者的手術(shù)時(shí)間、術(shù)后住院時(shí)間比較,差異均無統(tǒng)計(jì)學(xué)意義(P0.05);支撐板組患者的術(shù)后完全負(fù)重時(shí)間、骨折愈合時(shí)間少于空心釘組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。術(shù)后1年,支撐板組患者的AOFAS評(píng)分高于空心釘組,VAS評(píng)分和AO影像學(xué)評(píng)分低于空心釘組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。兩組患者并發(fā)癥發(fā)生率比較,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論后側(cè)支撐板固定治療脛骨后pilon骨折的臨床效果優(yōu)于前向后空心釘固定,其有助于提高術(shù)后骨折穩(wěn)定性,促進(jìn)關(guān)節(jié)功能的恢復(fù)。
[Abstract]:Objective to compare the rear support plate is fixed with the front to back clinical results of internal fixation for tibial Pilon fractures. Methods from February 2012 to February 2016 in our hospital for treatment of tibial Pilon fracture patients in 83 cases, according to the method of fixed support plate is divided into group 47 cases and cannulated screw group 36 cases. A comparative analysis of two groups of patients with operation time the postoperative hospitalization time, full weight-bearing time and healing time; using the American Orthopaedic Foot and Ankle Society (AOFAS) score, the evaluation function of 1 years postoperative ankle score VAS score and AO image; compared two groups of patients with postoperative complications. Results the operation time of the two groups of patients, postoperative hospital stay later, there were no significant differences (P0.05); support in the group of patients with postoperative full weight-bearing time, healing time is less than the hollow nail fracture group, the difference was statistically significant (P0.05). 1 years after the operation, the support plate group of patients with AOFA The S score is higher than the hollow nail group, VAS score and AO imaging score lower than the hollow nail group, the difference was statistically significant (P0.05). Two groups of patients complication rate comparison, the difference was not statistically significant (P0.05). Conclusion the clinical effect is better than the rear support plate fixation in the treatment of tibial Pilon fracture after the back hollow screw. It is helpful to improve the postoperative stability of fracture and promote the restoration of joint function.
